益气活血法治疗早期糖尿病肾病效果观察

Effect of Yiqi Huoxue therapy on early diabetic nephropathy

目的探讨益气活血法治疗早期糖尿病肾病(DN)的临床疗效。方法选择早期DN患者72例,随机分为观察组、对照组各36例,对照组采用常规西药和基础治疗,观察组联合口服益气活血中药制剂化瘀通脉颗粒,疗程8周。比较两组治疗前和治疗后8周中医证候积分,根据中医证候总积分计算总有效率。采用日立3700全自动生化分析仪及配套试剂检测两组治疗前和治疗后8周尿液中白蛋白排泄率(UAER)、血清胱抑素C(Cys C)水平,采用ELISA法检测血清TNF-α、TGF-β、IL-6水平。结果两组治疗后8周中医症状积分均低于治疗前,且观察组治疗后低于对照组(P均<0.05)。观察组总有效率高于对照组(P<0.05)。两组治疗后8周UAER、血清Cys C水平均较治疗前降低,且观察组治疗后低于对照组(P均<0.05)。两组治疗后8周血清TNF-α、TGF-β和IL-6水平均较前降低,且观察组治疗后低于对照组(P均<0.05)。结论益气活血中药制剂可进一步提高早期DN患者的临床效果,降低炎性因子水平。

Objective To investigate the clinical effect of Yiqi Huoxue therapy on the early diabetic nephropathy（ DN）. Methods Seventy-two patients with early DN were randomly divided into the observation group and control group with 36 cases in each. The patients in the control group were treated with conventional Western medicine and basic treatment,patients in the observation group received the oral Yiqi Huoxue traditional Chinese medicine（ TCM） preparation and Huayu Tongmai granule,and the course of treatment was 8 weeks. The TCM syndrome scores of the two groups before and8 weeks after treatment were compared,and the total effective rate was calculated according to the total score of TCM syndrome. The serum albumin excretion rate（ UAER） and serum cystatin C（ Cys C） levels of two groups before and 8 weeks after treatment were detected by Hitachi 3700 automatic biochemical analyzer and corresponding reagents. The levels of serum TNF-α,TGF-β,and IL-6 were detected by ELISA. Results The scores of TCM symptoms in the two groups after treatment for 8 weeks were all lower than those before treatment,and the observation group was lower than the control group after treatment（ P〈0. 05）. The total effective rate of the observation group was higher than that of the control group（ P〈0. 05）. The levels of UAER and Cys C at 8 weeks after treatment in the two groups were lower than those before treatment,and the observation group was lower than the control group after treatment（ P〈0. 05）. The serum TNF-α,TGF-β,and IL-6 levels of the two groups decreased after 8-week treatment,and the observation group was lower than the control group after treatment（ P〈0. 05）. Conclusion Yiqi Huoxue Chinese herbal preparation can further improve the clinical effect of early DN patients and reduce the level of inflammatory factors.
