尿酸与急性缺血性脑卒中短期预后的关系探讨 被引量:11

摘要 目的探讨尿酸与急性缺血性脑卒中短期预后的关系。方法选取新入院急性脑梗死病人346例,根据1个月后改良Rankin量表(mRS)评分将病人分为预后良好组(mRS 0分~1分)与预后欠佳组(mRS 2分~5分),采用Logistic回归分析研究预后不良的危险因素。根据入院时美国国立卫生研究院卒中量表(NIHSS)评分将病人分为NIHSS<5分组、NIHSS≥5分组及NIHSS<9分组、NIHSS≥9分组,比较组间高尿酸血症发生率。结果预后良好组病人高尿酸血症发生率明显低于预后欠佳组(P<0.05);将年龄<60岁、高血压、高脂血症、高尿酸血症、卒中类型为心源性脑栓塞(CE)和小动脉闭塞性卒中或腔隙性卒中(SAA)、NIHSS<9分作为自变量进行Logistic多元回归分析,NIHSS<9分、年龄<60岁为预后良好的保护因素,卒中类型CE为危险因素;且NIHSS高评分组高尿酸血症病人发生率均高于NIHSS低评分组(P<0.05)。结论脑卒中急性期尿酸水平不是脑梗死短期预后的独立预测因子,脑卒中急性期尿酸水平与脑卒中的严重程度相关。
