MCV、RDW联合网织红细胞参数在诊断贫血中的临床价值

Clinical value of MCV and RDW combined reticulocyte parameters in diagnosis of anemia

目的研究红细胞平均体积(MCV)、红细胞体积分布宽度(RDW)联合网织红细胞绝对值(Ret#)、网织红细胞百分率(Ret%)、低荧光强度网织红细胞百分率(LFR%)、中荧光强度网织红细胞百分率(MFR%)、高荧光强度网织红细胞百分率(HFR%)在贫血诊断中的临床效果。方法用LH750血细胞分析仪分别检测100例健康体检人员和再生障碍性贫血、缺铁性贫血和溶血性贫血各50例贫血患者的MCV、RDW、Ret#、Ret%、LFR%、MFR%、HFR%,并对其结果进行统计学分析。结果与健康组对比:三种贫血患者RDW、MFR%、HFR%均明显升高,LFR%明显降低;再生障碍性贫血组MCV明显升高,Ret#明显降低;缺铁性贫血组MCV明显降低,Ret%明显升高;溶血性贫血组Ret#、Ret%明显升高;差异均有统计学意义(P<0.01或P<0.05)。结论 MCV、RDW联合网织红细胞参数在临床中对鉴别和诊断不同类型贫血有重要价值,不可单纯靠一个指标来诊断。

Objective To study the clinical efficacy of mean corpuscular volume（MCV）,red blood cell volume distribution width（RDW）combined with reticulocyte absolute number（Ret#）,reticulocyte percentage（Ret%）,low fluorescence intensity reticulocyte percentage（LFR%）,medium fluorescence intensity reticulocyte percentage（MFR%）,high fluorescence intensity reticulocyte percentage（HFR%）in diagnosis of anemia. Methods Determination of MCV,RDW,Ret#,Ret%,LFR%,MFR% and HFR% was performed for 100 healthy individuals,50 cases of aplastic anemia,50 cases of iron deficiency anemia and 50 cases of hemolytic anemia by LH750 blood cell analyzer,the results were analyzed statistically. Results Compared with the healthy group：the RDW,MFR%,HFR% of the patients in the three anemia groups were significantly increased,the LFR% decreased obviously;the MCV in the aplastic anemia group was significantly increased and the Ret# was obviously decreased;the MCV in the iron deficiency anemia group was obviously reduced and Ret% obviously increased;the Ret# and Ret%increased obviously in the hemolytic anemia group;the differences were all statistically significant（P〈0.01 or P〈0.05）. Conclusion MCV and RDW combined with reticulocyte parameters have important value in identifying and diagnosing different types of anemia in clinic.It can＇t be diagnosed by only one single marker.
