摘要 目的:探讨多模式数字减影全脑血管造影(DSA)技术在脑血管疾病诊断中的临床应用价值。方法:对50例确诊为脑血管病患者于发病后1 h^5 d行多模式全脑数字减影血管造影检查,包括正侧位成像、双斜位成像、汤氏(瓦氏)位成像及选择性旋转3D重建成像,对颅内外动脉血管病变的部位进行分析。结果:多模式全脑DSA检查50例患者中脑动脉瘤16例,血管畸形6例,烟雾病1例,静脉窦血栓1例,血管狭窄病变23例,包括颈内动脉起始段狭窄10例,大脑中动脉狭窄9例,椎动脉及基底动脉狭窄4例;全脑数字减影可对脑血管疾病的病因诊断进行明确。结论:多模式数字减影全脑血管造影检查,可以确定患者病变部位的血流动力学改变、侧支代偿以及动静脉回流情况,对脑血管疾病治疗方案的选择具有重要的临床意义。 Objective The clinical value of multimode digital subtraction total brain angiography(DSA) in the diagnosis of cerebrovascular disease.Method In 50 cases of patients with cerebrovascular disease after onset in 1 h-5 d multi mode cerebral digital subtraction angiography,including lateral imaging,double oblique imaging,Tang' s(Darkbarbel) imaging and selective rotation 3 D reconstruction of cranial artery,vascular lesions and parts were analyzed.Results Multi mode whole brain DSA examination in 50 cases of patients with middle cerebral artery aneurysm in 16 cases,6 cases of vascular malformation,1 cases of moyamoya disease,1 cases of venous sinus thrombosis,vascular stenosis in 23 cases,including internal carotid artery stenosis in10 cases,9 cases of middle cerebral artery stenosis,vertebral artery and 4 cases of basilar artery stenosis;cerebral digital subtraction imaging can etiological diagnosis of cerebrovascular disease clearly.Conclusion Multi pattern digital subtraction angiography can determine the hemodynamic changes,collateral compensatory and arteriovenous reflux in the lesion,which has important clinical significance for the selection of cerebrovascular diseases.
