愈胆汤治疗胆囊切除术后综合征疗效及对胃肠激素的影响

Clinical Effect of Yudan Decoction in the Treatment of Postcholecystectomy Syndrome and Its Effect on the Levels of Gastrointestinal Hormones

目的探讨愈胆汤治疗胆囊切除术后综合征(PCS)的疗效及对血清胆囊收缩素(CCK)和胃动素(MTL)的影响。方法将97例PCS患者随机分为对照组(48例)和治疗组(49例)。两组患者均进行基础治疗,治疗组患者联用自拟愈胆汤治疗。两组患者疗程均为1个月,随访6个月。结果治疗组临床总有效率为91.84%,明显高于对照组的72.92%(χ~2=6.005,P=0.014<0.05);治疗后,两组患者临床症状评分,血清CCK和MTL水平均明显改善(P<0.05),且治疗组改善更显著(P<0.05);两组患者治疗期间均未出现严重不良反应;治疗组的复发率为2.22%(1/45),明显低于对照组的17.14%(6/35)(χ~2=5.489,P=0.019<0.05)。结论愈胆汤治疗PCS临床疗效显著,不仅可以改善临床症状,还能提高血清中CCK和MTL水平,安全性高,远期疗效好,值得临床推广。

Objective To investigate the effect of Yudan Decoction in the treatment of postcholecystectomy syndrome（ PCS） and its effect on the levels of serum cholecystokinin（ CCK） and motilin（ MTL）. Methods Totally 97 patients with PCS were randomly divided into the control group（ n = 48） and the treatment group（ n = 49）. The two groups were given basic treatment,on this basis,the treatment group was treated with Yudan Decoction. The two groups were treated for 1 month,and followed up for 6 months. Results The total effective rate of the treatment group was 91. 84%,which was significantly higher than 72. 92% of the control group（ χ~2 = 6. 005,P = 0. 014 0. 05）.After treatment,the scores of clinical symptom and levels of serum CCK and MTL in the two groups were significantly improved（ P〈0. 05）,and the improvement of the treatment group was more significant（ P〈0. 05）. No serious adverse reactions were observed in the two groups during treatment. The recurrence rate in the treatment group was 2. 22%（ 1/45）,which was significantly lower than 17. 14%（ 6/35） in the control group（ χ~2= 5. 489,P = 0. 019 0. 05）. Conclusion The therapeutic effect of Yudan Decoction in the treatment of PCS is significant. Yudan Decoction can improve the clinical symptoms,increase the levels of CCK and MTL in the serum,and it has high safety and good long-term effect,which is worthy of clinical promotion.
