静脉用药调配中心不合理用药医嘱分析及干预策略

Analysis and intervention strategy of irrational medication orders in pharmacy intravenous admixture services

目的统计并分析我院静脉用药调配中心(PIVAS)不合理的用药医嘱。方法调查我院PIVAS于2015年12月—2016年11月接收的482 056条输液医嘱,对不合理的医嘱进行登记和统计,并分析原因。结果在审核的482 056条静脉输注医嘱中,登记为不合理的医嘱共计1 277条,占医嘱总数的0.26%。经过分类统计,不合理医嘱主要有溶媒不当、药品配伍禁忌、给药剂量不当、给药频次不当、药品录入错误等方面的问题。结论药师审方可有效减少不合理的用药处方,纠正不合理医嘱,促进临床合理用药。

Objective To analyze the irrational prescriptions of pharmacy intravenous prescriptions checked by hospital pharmacyintravenous admixture services（PIVAS）.Methods Irrational medical orders among 482056 medical orders of PIVAS during Dec.2015 to Nov.2016 were collected for statistical analysis in respect of the types of the irrationality and its causes.Results 1277 medication orders were irrational and accounted for 0.26 % of the total medical orders,which mainly included irrational choice,drug incompatibility,irrational dose,irrational medication frequency and,etc.Conclusion Pharmacists can play an important role in revising the irrational medical advices in order to promote the rational drug use in the base of the problem of irrational medical advices in PIVAS.
