椎间融合器植骨融合对多节段脊髓型颈椎病的治疗效果 被引量:1

Effect of Interbody Fusion with Interbody Fusion Cage on the Physiological Curvature of Cervical Spine in Patients with Multiple Segmental Cervical Spondylotic Myelopathy

摘要 目的:探讨椎间融合器(cage)植骨融合对多节段脊髓型颈椎病患者的治疗效果。方法:将多节段脊髓型颈椎病患者90例均分成对照组和观察组,对照组行传统钛板固定术,观察组行cage植骨融合术,观察两组术前1 d及术后6个月时的脊髓功能、椎间高度及颈椎曲度;采用颈椎功能障碍指数量表(NDI)对颈椎功能(颈椎疼痛、提物、集中注意力、生活能力、睡眠、头疼)6个条目予以评估,并分析并发症发生率。结果:观察组脊髓功能恢复优良率高于对照组,组间比较差异有统计学意义(P<0.05);观察组术后6个月的椎间高度、颈椎曲度高于对照组,差异有统计学意义(P<0.05);观察组术后6个月的颈椎疼痛、提物、头疼评分低于对照组,组间比较差异有统计学意义(P<0.05);观察组术后并发症发生率与对照组比较,差异无统计学意义(P>0.05)。结论:cage植骨融合术能促进多节段脊髓型颈椎病患者颈椎曲度的恢复,减轻颈脊髓功能障碍。 Objective: To investigate the effect of bone graft fusion with interbody fusion cage on patients with multilevel cervical spondylotic myelopathy. Methods: 90 cases with cervical spondylotic myelopathy were randomly divided into the control group ( n =45) and the observation group ( n =45). In the control group, the traditional titanium plate fixation was performed. And the observation group received bone graft fusion with interbody fusion cage. The spinal cord recovery effect and intervertebral height and cervical curvature change of both groups 1 d before operation and 6 months after operation. The cervical vertebral function was assessed by the neck disability index scale (NDI), and the incidence of complications was analyzed. Results: The recovery excellent rate of spinal cord function in the observation group was 93.33%, higher than that of the control group (77.78%), and there was a statistically significant difference between the two groups (P 〈0.05). The intervertebral height and cervical curvature of the observation group were significantly higher than those of the control group 6 months after operation ( P 〈0.05).The scores of cervical pain, extraction and headache in the observation group 6 months after operation was lower than that in the control group, and there was a significant difference between the two groups ( P 〈0.05).The complication rate of the observation group was 4.44%, and it was 11.11% in the control group, without statistical significance ( P 〉0.05). Conclusion: Bone graft fusion with interbody fusion cage can promote the recovery of cervical curvature of patients with multiple segments of cervical spondylosis, and reduce the dysfunction of cervical vertebra.
