经阴道四维子宫输卵管超声造影在不孕症诊断中的应用价值

Application value of transvaginal four-dimensional hysterosalpingography in the diagnosis of infertility

目的探究经阴道四维子宫输卵管超声造影(TVS4D-Hy Co Sy)在不孕症诊断中的应用效果。方法选择2017年1~6月在我院就诊的100例不孕症患者作为研究对象,所有患者均行TVS4D-Hy Co Sy检查,并与腹腔镜下通染液检查结果进行比较。结果检查后发现100例不孕症患者中,共检出200条输卵管。TVS4D-Hy Co Sy检查显示,输卵管通畅110条,输卵管通而不畅60条;输卵管完全阻塞30条;腹腔镜下通染液检查显示,输卵管通畅112条,输卵管通而不畅60条;输卵管完全阻塞28条。以腹腔镜下通染液检查为标准,TVS4D-Hy Co Sy检查输卵管通畅的符合率为98.21%,通而不畅符合率为100.00%,完全阻塞的符合率为92.86%。两种检查方法比较,差异无统计学意义(P>0.05);在检查过程中,TVS4D-Hy Co Sy检查6例出现明显疼痛情况,1例存在轻微疼痛,不良反应发生率为7.00%,相较于腹腔镜下通染液检查,差异无统计学意义(P>0.05)。结论采用TVS4D-Hy Co Sy对不孕症患者进行检查,可有效显示其子宫输卵管的具体形态,并可对其通畅性进行准确判断,属于临床较有效的检查方法,值得推广。

Objective To explore the application of transvaginal four-dimensional hysterosalpingography（TVS 4 D-HyCo Sy） in the diagnosis of infertility.Methods A total of 100 patients with infertility who were treated in our hospital from January to June in 2017 were selected as the research subjects.All patients were preformed with TVS 4 D-HyCoSy examinations and the results were compared with those using laparoscopic dye-fluid test.Results A total of 200 fallopian tubes were detected in 100 infertile patients after examinations.TVS 4 D-HyCoSy examination showed that 110 tubes in patency,60 tubes in patency but not smooth,and the remaining 30 tubes in complete obstruction.Laparoscopic dyefluid examination indicated that the number of tubal patency,patency but not in total,and complete obstruction was112,60,and 28.Set the laparoscopic dye-fluid examination as standard,the accordance rate of TVS 4 D-HyCoSy in detecting tubal patency was 98.21%,and 100.00% for patency but not in total,and 92.86% for complete obstruction.The difference between the two examinations was not significant（P〈0.05）.During the examination,there were 6 cases occurred with obvious pain and 1 case with mild pain in the TVS 4 D-HyCoSy examination.The incidence of adverse reactions was 7.00%.Compared with laparoscopic dye-fluid examination,the difference was not significant（P〈0.05）.Conclusion The use of TVS 4 D-HyCoSy in the examination of infertility patients can effectively display the specific shapes of the uterus and fallopian tube,and can accurately determine its patency,which is an effective clinical examination method and is worthy of promotion.
