高频超声成像对胎盘植入的诊断价值研究

摘要 目的探讨对胎盘植入诊断应用高频超声成像的临床价值。方法选取2016年2月~2018年2月我院收治的50例接受胎盘植入诊断的患者作为研究对象,进行回顾性分析,分别采用低频、高频超声成像检测,分析检测结果情况。结果病理结果检出47例患者胎盘植入,检出率94.00%。低频结合高频检出44例,检出率88.00%,灵敏度91.49%,特异度66.67%,阳性预测值97.73%,阴性预测值33.33%,假阳性率33.33%,假阴性率8.51%,诊断符合率90.00%。低频检出37例,检出率74.00%,灵敏度74.46%,特异度33.33%,阳性预测值94.59%,阴性预测值7.69%,假阳性率66.67%,假阴性率25.53%,诊断符合率72.00%。低频结合高频检测的特异度、灵敏度、符合率均明显高于单独低频检测结果,差异有统计学意义(P<0.05)。结论对胎盘植入诊断患者采取高频超声成像辅助诊断,可有效提高诊断特异度及灵敏度,并降低假阴性以及假阳性的发生率,有较高临床应用价值。 Objective To investigate the diagnostic value of high frequency ultrasound imaging for placental implantation.Methods From February 2016 to February 2018,50 patients with placenta implantation diagnosed in our hospital were reviewed and analyzed retrospectively.Low frequency and high frequency ultrasound imaging were used,and the results of the test were analyzed.Results Pathological examination revealed placenta implantation in 47 patients,with a detection rate of 94.00%.The detection rate of low frequency combined with high frequency was 88.00%,the sensitivity was 91.49%,the specificity was 66.67%,the positive predictive value was 97.73%,the negative predictive value was33.33%,the false positive rate was 33.33%,the false negative rate was 8.51% and the diagnostic coincidence rate was90.00%.The detection rate of Low frequency detection was 74.00%,the sensitivity was 74.46%,the specificity was33.33%,the positive predictive value was 94.59%,the negative predictive value was 7.69%,the false positive rate was66.67%,the false negative rate was 25.53% and the diagnostic coincidence rate was 72.00%.The specificity,sensitivity and coincidence rate of low frequency combined with high-frequency detection were significantly higher than those of low frequency detection alone,with a statistical difference(P〈0.05).Conclusion For the diagnosis of placenta accrete,high frequency ultrasound imaging can improve the diagnostic specificity and sensitivity,reduce the incidence of false negative and false positives,and has high clinical application value.
