西药房调剂差错处方原因及对策研究

目的探讨我院西药房调剂差错处方发生的原因,提出相关对策。方法统计2015年—2017年我院各年西药房调剂差错处方,并分析原因及对策。结果各科室中心脏内科调剂差错最多(14方);差错药品以口服药为主,占66.66%;引起差错的因素主要与药物品种有关,共有36例(占41.86%)。结论我院西药房调剂差错有配药品种出错、药品数量出错、患者错领、用法与用量差错及用药禁忌5种类型。针对我院近3年处方差错情况,应加强系统管理,增强医师的专业素养和用药水平,加强药师工作责任心和配药技巧,引进新技术完善处方审核管理系统,保证调剂处方质量,防止差错发生。

Objective To study the causes of data error prescription in our hospital pharmacy and put forward relevant countermeasures. Methods Statistics our hospital pharmacy errors in each of the prescription from 2015 to2017, and analysis of causes and gone wrong prescription from a detailed analysis of various aspects, and according to the error to explore corresponding countermeasures. Results Most heart internal medicine dispensing errors in each department（14）,Wrong medicine is given priority to with oral medications, accounting for 66.66%; The factors that caused the errors were mainly related to the drug variety in 36 cases（41.86%）.Conclusion Our hospital pharmacy dispensing errors in wrong with varieties of drugs, drug quantity, collar, usage and dosage error of patients and contraindication five types. For nearly three years prescription errors in our party, summary should strengthen system management, strengthen doctor＇s professional accomplishment and medical level, strengthen the pharmacist dispensed work sense of responsibility and skill, introduce new technology to improve the prescription audit management system,guarantee the quality of dispensing prescription, to prevent any mistakes.
