冰硝散外敷配合涌泉穴艾灸缓解阳虚水泛型心力衰竭患者下肢水肿

External application of ice nitrate dispersion combining with moxibustion at Yongquan acupoint alleviates lower limb edema for patients with yang deficiency and water generic heart failure

目的观察冰硝散外敷配合涌泉穴艾灸对阳虚水泛型心力衰竭患者下肢水肿的临床效果。方法将阳虚水泛型心力衰竭水肿患者54例随机分为A组(18例)、B组(17例)、C组(19例),三组均常规行利尿、强心、扩血管等基础治疗,A组行常规护理;B组给予冰硝散外敷;C组行冰硝散外敷的同时加艾灸涌泉穴。观察三组干预前及干预1、3周后小腿周径的变化。结果三组不同时间小腿周径比较,干预主效应、时间主效应及交互效应差异有统计学意义(均P<0.01),B、C两组干预后小腿周径的改善优于A组,且C组干预效果优于B组(均P<0.05)。结论冰硝散外敷配合涌泉穴艾灸较单纯冰硝散外敷及常规护理更能有效改善阳虚水泛型心力衰竭患者的下肢水肿。

Objective To observe the clinical effect of ice nitrate dispersion external application combining with moxibustion at Yongquan acupoint on alleviation of lower limb edema for patients with yang deficiency and water generic heart failure.Methods Totally 54 edema patients with yang deficiency and water generic heart failure were randomized into group A（n=18）,group B（n=17）and group C（n=19）,the three groups routinely received diuretics,cardiac drugs,vasodilators and other basic therapy.Group A was given traditional nursing,and group B additionally received external application of ice nitrate dispersion,while group C received moxibustion at Yongquan acupoint based on group B.The changes on peripheral diameter of the calf of the three groups before and 1 and 3 weeks after the intervention were observed.Results Compared the peripheral diameter of the calf at different time points among the three groups,the differences in the intervention main effect,time main effect and interaction main effect were statistically significant（P〈0.01 for all）.The improvement of peripheral diameter of the calf in group B and C were better than that in group A,and the intervention effect of group C was better than that of group B（P〈0.05 for all）.Conclusion External application of ice nitrate dispersion combining with moxibustion at Yongquan acupoint can alleviate lower limb edema better for patients with yang deficiency and water generic heart failure,when compared with routine nursing and only external application of ice nitrate dispersion.
