品管圈在降低神经内科鼻胃管非计划拔管中的应用

Application of Quality Control Circle in Reducing Unplanned Extubation of Nasogastric Tube in Neurology Department

目的为了减少神经内科鼻胃管非计划拔管率并提高护理质量,分析和研究采用品管圈的价值和意义。方法随机从2016年11月—2017年11月来该院神经内科治疗并留置鼻胃管的患者中抽取76例作为研究对象,应用计算机软件将其平均分组,其中38例患者接受常规护理措施并设为对照组,余38例患者接受品管圈活动并设为观察组,比较两组患者在鼻胃管非计划拔管率、护理缺漏率以及留置鼻胃管的工作时数和护理成本等方面的差异。结果研究数据显示,观察组患者的鼻胃管非计划拔管率以及护理缺漏率依次为10例(26.32%)、3例(7.89%),对照组患者的鼻胃管非计划拔管率以及护理缺漏率依次为28例(73.68%)、20例(52.63%),组间比较差异有统计学意义(P<0.05);观察组患者留置鼻胃管的工作时数和护理成本依次为(7.47±2.55)min/人、(7.32±2.55)元/人,而对照组患者留置鼻胃管的护理用时和护理成本依次为(13.26±10.75)min/人、(11.85±9.05)元/人,组间比较差异有统计学意义(P<0.05)。结论神经内科留置鼻胃管患者接受品管圈管理,不仅能够减少护理成本,缩短护理时间,还能够减少鼻胃管非计划拔管率,效果显著,值得推广。

Objective In order to reduce the unplanned extubation rate and improve the quality of care in the nasogastric tube of neurology department, the value and significance of adopting the quality control circle were analyzed and studied.Methods 76 patients randomly selected from the Department of Neurology in the hospital for treatment and indwelling na-so-gastric tube from November 2016 to November 2017 were randomly selected and grouped by computer software. Among them, 38 patients received routine nursing measures as the control group, the remaining 38 patients received quality control and were placed in the observation group. The unplanned nasogastric tube extubation rate, carelessness rate, and the number of working hours and nursing costs for the nasogastric tube were compared between the two groups. Results The study data showed that the unplanned extubation rate and nursing carelessness rate of nasogastric tube in the observation group were 10 cases（26.32%） and 3 cases（7.89%）, and the rate of non-planned extubation of the nasogastric tube and the rate of carelessness of the control group followed by 28 cases（73.68%）,20 cases（52.63%）, the difference between the groups was statistically significant（P0.05）; the working hours and nursing costs of the patients in the observation group were（7.47±2.55）min/person,（7.32 ±2.55）yuan/person, while the care time and nursing cost for the control group for indwelling nasogastric tube were（13.26 ±10.75）min/person and（11.85 ±9.05）yuan/person respectively. The difference was statistically significant（P 0.05）.Conclusion Neurosurgery indwelling nasogastric tube patients receiving quality management, not only can reduce the cost of care, shorten the time of care, but also can reduce the nasogastric tube unplanned extubation rate, the effect is significant, it is worthy of promotion.
