摘要
目的:研究前哨淋巴结中缺氧相关蛋白碳酸酐酶-9(carbonic anhydrase-9,CA-9)的表达与早期乳腺癌淋巴结转移的关系。方法:选取2013年1月-2017年12月在本院全程治疗的乳腺癌患者46例,运用免疫组化的方法检测患者前哨淋巴结石蜡标本中的CA-9的表达,分析CA-9与早期乳腺癌临床病理因素的关系。结果:46例乳腺癌中,前哨淋巴CA-9阳性率为19.57%(9/46);其中淋巴结有转移的CA-9阳性率为88.89%(8/9),明显高于淋巴结无转移的2.7%(1/37),比较差异有统计学意义(P<0.05);临床分期Ⅱb、Ⅱa、Ⅰ、0期的CA-9阳性率分别为100%(6/6)、13.64%(3/22)、0、0,比较差异有统计学意义(P<0.05);但CA-9阳性与患者月经状况、原发肿瘤大小、病理类型、ER、PR、Cerb B-2、Ki-67无关(P>0.05)。结论:前哨淋巴结中CA-9的表达与早期乳腺癌的腋窝淋巴结转移相关,其表达随临床分期的升高而明显增加,但与患者月经情况、肿瘤大小、病理类型、ER、PR、Cerb B-2、Ki-67无关。
Objective:To investigate the relationship between carbonic anhydrase-9(CA-9) expression in sentinel lymph nodes and early breast cancer lymph node metastasis.Method:A total of 46 patients with breast cancer who were treated in our hospital from January 2013 to December 2017 were selected,the expression of CA-9 in the paraffin samples of sentinel lymph nodes was detected by immunohistochemistry,the relationship between CA-9 and the clinicopathological factors of early breast cancer were analyzed.Result:In 46 cases of breast cancer,the positive rate of sentinel lymphatic CA-9 was 19.57%(9/46).Among them,the positive rate of CA-9 with lymph node metastasis was 88.89%(8/9),which was significantly higher than 2.7%(1/37) of lymph nodes without metastasis,the difference was statistically significant(P〈0.05).The positive rate of CA-9 in clinical stage Ⅱb,Ⅱa,Ⅰ and 0 was respectively 100%(6/6),13.64%(3/22),0 and 0,the difference was statistically significant(P〈0.05).But positive CA-9 was not associated with the patients' menstrual status,primary tumor size,pathological type,ER,PR,Cerb B-2 and Ki-67(P〉0.05).Conclusion:The expression of CA-9 in sentinel lymph nodes was correlated with axillary lymph node metastasis of early stage breast cancer,and its expression was significantly increased with the increase of clinical stage,but it was not related to the patients' menstrual situation,tumor size,pathological type,ER,PR,Cerb B-2 and Ki-67.
