乳腺黏液腺癌患者的超声影像特征研究

目的研究乳腺黏液腺癌患者的超声影像特征。方法乳腺黏液腺癌患者45例,回顾性分析45例患者的超声影像图,分析超声影像图与患者病理特征的关系。结果 45例乳腺黏液腺癌患者中混合型22例,单纯型23例,混合型患者均是浸润性导管癌与乳腺黏液腺癌同时存在,其中乳腺黏液腺癌所占比例范围2%~97%。45例乳腺黏液腺癌患者中,75.56%患者肿块边界清晰,37.78%患者肿块形态较规则,44.44%患者肿块形态欠规则,8.89%患者肿块纵横比≥1,48.89%患者肿块内部回声均匀,62.22%患者血流信号无血流,20.00%为点状血流,82.22%患者后方回声增强,75.56%患者无点状钙化,51.11%患者内部回声不均匀,75.56%患者肿块内部无液化,86.67%患者无淋巴结转移,单纯型乳腺黏液腺癌患者肿块边界较混合型患者清晰,且单纯型患者血流信号无血流比例较混合型高,条状血流比例较混合型低;单纯型患者淋巴结转移比例较混合型低,差异有统计学意义(P<0.05)。结论单纯型乳腺黏液腺癌较混合型主要表现为边界清晰,血流信号较少,肿瘤后方回声增强,单纯型乳腺黏液腺癌患者多表现为良性肿瘤特征,易于误诊为乳腺良性肿瘤,而混合型乳腺黏液腺癌患者超声影像多表现为恶性肿瘤超声特征。

Objective To study the characteristics of ultrasound imaging in patients with breast mucinous adenocarcinoma. Methods 45 patients with breast mucinous adenocarcinoma were retrospectively analyzed. Ultrasonographic images of 45 patients were analyzed retrospectively,and the relationship between them was analyzed. Results 45 cases of mixed breast mucinous adenocarcinoma in 22 cases,23 cases of pure type,mixed type of patients were invasive ductal carcinoma and breast mucinous adenocarcinoma at the same time,of which the proportion of breast mucinous adenocarcinoma range of 2% to 97%. 45 cases of mucinous breast cancer patients,75. 56% patients with circumscribed mass,37. 78% patients with tumors had regular shape,44. 44% patients with tumors with irregular shape and aspect ratio of more than 1 patients with 8. 89% masses,48. 89% masses with internal echo uniform,62. 22% patients with no blood flow signal,20% point blood flow,82. 22% patients with posterior echo enhancement,75. 56% patients no calcification,51. 11% patients with uneven internal echo,internal 75. 56% patients without tumor liquefaction,without lymph node metastasis in 86. 67% patients,pure type mucinous adenocarcinoma of the breast tumor patients with mixed type patients with clear boundary,and the simple type of blood flow in patients with no blood flow signal was mixed high,blood flow ratio was mixed low; simple type the proportion of patients with lymph node metastasis than mixed type low（ P 〈 0. 05）. Conclusion Simple type of breast mucinous adenocarcinoma mixed mainly manifested as a clear boundary,less blood flow signal,enhanced echo in the posterior tumor,simple breast cancer with mucinous adenocarcinoma,and benign breast cancer. Ultrasonographic features of mixed breast mucinous adenocarcinoma are malignant tumor ultrasonographic features.
