低温等离子消融术与开窗减压术治疗牙源性肿瘤的疗效对比

Comparison of the Effect of Low Temperature Plasma Ablation and Fenestration Decompression in the Treatment of Odontogenic Tumors

目的比较低温等离子消融术与开胃减压术治疗牙源性肿瘤的临床疗效。方法将45例牙源性肿瘤患者根据手术方式分为对照组(n=20例)和研究组(n=25例),对照组行开窗减压术,研究组行低温等离子消融术。比较2组的临床效果,随访12~36个月,统计2组术后复发转移情况及生活质量。结果研究组的治疗总有效率为92.00%,明显高于对照组的75.00%(χ~2=10.488,P<0.05)。研究组术后囊肿结构消失时间、术后骨密度恢复时间、二次手术时间均明显短于对照组(P<0.05)。研究组有5例患者在术后12个月左右,行刮除术去除囊肿组织,对照组所有患者在术后12~36个月均行刮除术去除囊壁组织,研究组二次手术发生率显著低于对照组(P<0.05)。2组术前各方面相比差异均无统计学意义(P>0.05),2组术后在疼痛、外貌、情感方面评分与术前相比明显升高,并且研究组评分显著高于对照组(P<0.05);2组术后在味觉、唾液、咀嚼方面评分相比,差异无统计学意义(P>0.05)。结论低温等离子消融术疗效显著,利于患者恢复,能够改善患者术后生活质量。

Objective To compare the clinical efficacy of low-temperature plasma ablation and fenestration decompression in the treatment of odontogenic tumors. Methods 45 cases of odontogenic tumor patients treated in our hospital were considered as objects. According to the mode of operation,the patients were divided into the control group（ n = 20 cases） and the study group（ n = 25 cases）; the control group underwent fenestration decompression; the study group received low-temperature plasma ablation. The intraoperative,postoperative were compared between the 2 groups. Follow-up,12 ~ 36 months,statistics of the 2 groups of postoperative recurrence and metastasis after operation and the quality of life were compared. Results The total effective rate of the study group was 92. 00%,significantly higher than the control group 75. 00%（ χ~2= 10. 488,P 〈 0. 05）. The cyst structure disappeared time after operation,the bone density of postoperative recovery time,time of second operation in the study group were significantly shorter than the control group（ P 〈 0. 05）. In 12 months after operation,the study group of 5 patients were treated with curettage removal in cyst tissue,and all patients in the control group in 12 ~ 16 months after operation were treated with curettage to remove wall tissue. The second surgery incidences of the study group was significantly lower than the control group（ P 〈 0. 05）. The differences were not statistically significant（ P 〉 0. 05） in preoperative. Compared with the preoperative,two groups of pain,emotional,appearance were increased significantly in postoperative,and the study group were significantly higher than those in control group（ P 〈 0. 05）; the two groups after operation in taste,saliva,chewing score had no significant difference（ P 〉 0. 05）. Conclusion The effect of low temperature plasma ablation is significant,it is beneficial to the recovery of the patients. And low temperature plasma ablation can improve the quality of life after the operation.
