彩色多普勒超声评估肝癌周围血管受侵犯程度及可切除性的价值

The Value of Color Doppler Ultrasound in Assessing the Degree of Peripheral Blood Vessels Invasion and Resectability in the Liver Cancer

目的探讨彩色多普勒超声评估肝癌周围血管受侵犯程度及可切除性的价值。方法以肝癌患者2600例作为研究对象,所有患者行彩色多普勒超声检查,评估肝癌周围血管受侵犯程度及可切除性,同时进行临床与病理分析。结果彩色多普勒超声显示肝癌周围血管受侵犯400例,发生率为15.4%;超声评估肝癌周围血管受侵犯的敏感性与特异性为95.0%和99.9%。超声诊断为肝动脉变异40例,超声评估肝癌患者肝动脉变异的敏感性与特异性为95.2%和100.0%。超声评估肝癌可切除280例,彩色多普勒超声评估肝癌可切除性的敏感性与特异性为98.6%和99.8%。结论彩色多普勒超声可显示肝癌周围血管受侵犯程度与肿瘤供血血管,有利于指导判定肝动脉变异情况与手术可切除性,为辅助临床诊治提供参考。

Objective To evaluate the value of color Doppler ultrasound in assessing the degree of invasion and resectability of the peripheral blood vessels of liver cancer. Methods 2600 cases of liver cancer patients were collected as the research object,all patients were given color Doppler ultrasound assessment of degree of peripheral blood vessels invasion and resectability,and were given the clinical and pathological analysis. Results Color Doppler ultrasonography showed that there were 400 cases of peripheral blood vessels,the incidence was 15. 4%. The sensitivity and specificity of ultrasound to evaluate the peripheral blood vessels invasion were 95% and 99. 9% respectively. And there were 40 cases of hepatic artery variation diagnosed by ultrasound,and the sensitivity and specificity of ultrasonic assessment of hepatic artery variation in patients with liver cancer were 95. 2% and100%. There were 280 cases of liver cancer were resectable by ultrasound,and the sensitivity and specificity of color Doppler ultrasound were 98. 6% and 99. 8% in evaluating the resectability of liver cancer. Conclusion The color Doppler ultrasonography can show the degree of invasion of blood vessels and the blood supply vessels of the liver cancer,which is helpful for guiding the judgement of the variation of the hepatic artery and the resectability of the operation,so as to provide references for clinical diagnosis and treatment.
