摘要
目的探讨人工流产次数与高危型人乳头瘤病毒感染(HPV)和宫颈癌发生的关系。方法选择宫颈癌患者769例作为观察组,同期选择非宫颈癌及宫颈上皮内瘤变患者769例作为对照组,两组都进行高危型HPV检测,同时调查患者的人工流产次数、一般资料并进行相关性分析。结果观察组中人工流产0次69例,1~2次200例,≥3次500例;对照组中人工流产0次426例,1~2次300例,≥3次43例,两组对比差异都有统计学意义(P<0.05)。观察组与对照组高危型HPV阳性率分别为96.5%和21.6%,观察组高危型HPV阳性率明显高于对照组(P<0.05)。多因素非条件logistic回归分析显示高危型HPV阳性率、人工流产次数、配偶包皮过长、开始性生活年龄早为导致宫颈癌发生的主要独立危险因素(P<0.05)。直线相关分析观察组的高危型HPV阳性率与人工流产次数呈现明显正相关性(γ=0.344,P<0.05)。结论宫颈癌的发生是多种因素共同作用的结果,反复人工流产以及高危型HPV感染可诱发宫颈癌的发生。
Objective To investigate the relationship between the number of artificial abortion and high risk of human papillomavirus( HPV) infection and cervical cancer. Methods 769 cases of cervical cancer patients were selected as the observation group,and the other 769 patients with non cervical cancer and cervical intraepithelial were selected as the control group at the same period. Both groups recived high-risk HPV detection,and the artificial abortion,general information were given investigation and correlation analysis. Results In the observation group,the 0 times of abortion was 69 cases,1 ~ 2 times of abortion was200 cases,≥3 times of abortion was 500 cases; 0 times of abortion was cases,1 ~ 2 times 300 cases and ≥3 times 43 cases in the control group,There were statistically significant differences between the 2 groups( P 〈 0. 05). The positive rate of high risk HPV in the observation group and the control group were 96. 5% and 21. 6%,respectively,the positive rate of high risk HPV in the observation group was significantly higher than that of the control group( P 〈 0. 05). Multivariate unconditional logistic regression analysis showed that the positive rate of high-risk HPV,number of artificial abortion,the redundant prepuce and the age of sexual life were the main independent risk factors for cervical cancer( P 〈 0. 05). The correlation analysis showed that the positive rate of high-risk HPV was positively related to the number of artificial abortion( γ = 0. 344,P 〈 0. 05). Conclusion The cervical cancer is the result of multiple factors,the repeated artificial abortion and high risk HPV infection can cause cervical cancer.
