摘要
目的探讨乙型肝炎(hepatitis B virus,HBV)患者血清中游离mt DNA含量与其患肝癌(hepatocellular carcinoma,HCC)的风险相关性,为临床早期诊断HCC提供理论和实验依据。方法收集解放军第153中心医院156例HBV相关的HCC患者(试验组)和312例非肝癌HBV患者(对照组)基本信息、临床资料、血液样本,通过实时荧光定量PCR检测血清中游离mt DNA含量。比较两组mt DNA含量的差异性。通过病例对照研究对血清中游离mt DNA含量与HBV患者患HCC的风险相关性进行分析,对血清中mt DNA含量与患HCC风险的相关性进行单因素和多因素的逻辑回归模型分析。通过分层分析评估在不同特征人群中游离mt DNA含量与HBV相关的HCC发病风险之间的相关性。结果试验组患者的mt DNA含量低于对照组,年长患者mt DNA含量较年轻患者mt DNA含量低,mt DNA含量与HBV相关的HCC风险的相关性在年轻患者、无肝硬化患者中表现较为显著:年轻患者中mt DNA含量低的患者其患HCC的风险相对较高;无肝硬化的患者中mt DNA含量低的患者比mt DNA含量高的患者患HCC的风险显著增加。结论年轻患者、无肝硬化HBV患者中,血清中游离mt DNA含量降低,HBV相关HCC的发病风险增加。
Objective To investigate the association between circulating cell-free mtRNA content and HBV-related hepatocelluar carcinoma(HCC) risk in hepatitis B virus(HBV) patients,to provide theoretical and experimental basis for clinic. Methods One hundred and fifty-six HBV-related HCC patients(experiment group) and 312 cancer-free HBV controls(control group) were selected. The mtDNA content in serum DNA samples was detected by PCR. The association between mtDNA content and HCC risk was evaluated by Logistical regression model using univariate and multivariate analyses. The effect of mtDNA content and some current clinical parameters were analyzed. Results The mtDNA content in the experiment group was lower than that in the control group. The correlation between the mtDNA content and the HCC risk associated with HBV was more significant among the young patients and patients with no cirrhosis. The risk of HCC was relatively higher in young patients with low mtDNA content. Patients with low level of mtDNA had a significantly higher risk of developing HCC than those with high level of mtDNA. Conclusion In young patients without cirrhosis,by serum free mtDNA levels decreasing,the risks of HBV-associated liver cancer increase.
