胎盘蛋白13促进妊娠期高血压疾病患者血管损伤修复的作用及机制研究 被引量:14

摘要 目的:探讨胎盘蛋白13(PP13)维持妊娠期高血压疾病(HDCP)患者内皮祖细胞(EPC)功能的作用及机制。方法:收集2014年1月至2017年10月于青岛市妇女儿童医院产科就诊的10例HDCP患者,同期选取10例正常妊娠者作为对照组。流式检测CD34+VEGFR-2+EPC比例;ELISA法检测PP13、VEGF和SDF-1浓度;Transwell实验检测EPC迁移功能。结果:正常妊娠组和HDCP组的CD34+VEGFR-2+EPC比例分别为(0.5288±0.02897)%和(0.1217±0.01950)%,外周血中PP13浓度分别为(146.1±1 2.66)pg/ml和(38.08±8.144)pg/ml,两组比较差异均有统计学意义(P<0.0001)。相关性分析结果示,妊娠患者外周血EPC比例和PP13浓度呈明显正相关(r=0.9113,P<0.0001)。动物实验结果示,PP13显著促进EPC动员,相对于对照组,EPC比例上升了0.654倍(P=0.0199);PP13促进HDCP患者受损EPC的迁移和旁分泌功能,相对于HDCP组,EPC迁移率上升了0.901倍(P=0.0030),培养上清中VEGF和SDF-1浓度分别上升了3.58倍和2.94倍(P=0.0062,P=0.0122)。结论:妊娠期患者外周血中PP13和EPC呈正相关,PP13具有促进HDCP患者EPC动员和受损EPC功能修复的作用。本研究为PP13在HDCP的预防和治疗中的作用提供了理论依据。 Objective: To study the effect and mechanism of placental protein 13( PP13) to maintain the function of endothelial progenitor cells( EPC) in patients with hypertension disorders complicating pregnancy( HDCP).Methods: 10 cases of HDCP patients and 10 cases of normal pregnancy were selected who were admitted to the Obstetrics Department of Qingdao Women and Children's Hospital from Jan.2014 to Oct.2017.Flow cytometry was used to detect the proportion of CD34+VEGFR-2+ EPC.ELISA was used to detect the concentrations of PP13,VEGF and SDF-1. Transwell test was used to detect the migration of EPC. Results: The proportion of CD34+VEGFR-2+EPC in peripheral blood of control patients and HDCP patients was( 0.5288±0.02897) % and( 0.1217±0.01950) %,the concentration of PP13 was( 146.1 ±12.66) pg/ml and( 38. 08 ± 8. 144) pg/ml,the difference was statistically significant( P 〈0.0001). There was a significant positive correlation between the proportion of EPC and the concentration of PP13 in the peripheral blood of pregnant women( r = 0.9113,P〈0.0001).Animal experiments showed that PP13 could significantly promote the mobilization of EPC,compared with the control group,the proportion of EPC increased by 0. 654 times( P = 0. 0199).PP13 promoted the migration and paracrine function of EPC in HDCP patients,compared with the HDCP group,the EPC mobility increased by 0.901 times( P = 0.0030),the concentration of VEGF and SDF-1 in the culture supernatant increased by 3.58 times and 2.94 times respectively( P = 0.0062,P = 0.0122).Conclusion: PP13 and the proportion of EPC has a positive correlation in peripheral blood of patients with pregnancy,and PP13 has the role of promoting EPC mobilization and impaired EPC function repair in HDCP patients.Our research provides a theoretical basis for the prevention and treatment of PP13 in HDCP.
