螺旋CT增强扫描评价食管癌分化程度的价值

The value of spiral CT enhanced scan in the evaluation of pathological grades of esophageal carcinoma

目的探讨螺旋CT增强扫描评估食管癌病理分化程度的价值。方法回顾分析81例经术后病理证实的食道癌患者临床资料,依据病理分化程度分为高、中、低分化组。分别于CT平扫及增强扫描测量病灶CT值,并计算病灶强化程度。采用单因素方差分析比较不同分化程度食管癌强化程度的差异。结果高、中、低分化食管癌患者的静脉期强化程度分别为(28.75±14.89)、(39.00±15.06)、(44.58±14.13)Hu,差异有统计学意义(P<0.05);其中高分化与低分化、中分化与低分化食管癌的强化程度差异有统计学意义(P<0.05)。结论螺旋CT增强扫描对食管癌病理分化程度的评估有重要临床价值。

Objective To investigate the clinical value of spiral CT enhanced scan in evaluating the pathological grade of esophageal carcinoma. Methods The clinical data about 81 patients with esophageal carcinoma who were identified by postoperative pathological examination were retrospectively analyzed. according to the pathological differentiation degree,these cases were divided into three groups： well differentiation group,moderate differentiation group and poor differentiation group. The CT value of esophageal lesions was measured by CT plain scan and enhanced venous phase scan,respectively,and the enhancement degree of disease focus was calculated. Moreover the correlation between CT enhancement degree and pathological differentiation grade of esophageal lesion was analyzed statistically. Results The enhancement degrees of venous phase in well differentiation group,moderate differentiation group and poor differentiation group were（ 28. 750 ± 14. 89）,（ 39. 00 ± 15. 06）,（ 44. 58 ± 14. 13） Hu,respecyively,there was significant difference among the three grousp（ P 〈 0. 05）,moreover there was significant difference in the enhancement degree between well differentiation group and poor differentiation group and between well differentiation group and moderate differentiation group（ P 〈 0. 05）. Conclusion The spiral CT enhanced scan has important clinical value in evaluating the pathological differentiation degree of esophageal carcinoma.
