速效牙痛宁酊配合常规疗法对根管治疗后牙髓炎患者龈沟液白介素、MMP-8/TIMP1含量的影响

Effects of quick-impact toothache tincture combined with routine therapy on interleukin and MMP-8/TIMP1 levels in gingival crevicular fluid od patients with pulpitis after root canal therapy

目的探讨速效牙痛宁酊配合常规疗法对根管治疗后牙髓炎患者龈沟液白细胞介素、基质金属蛋白酶8(MMP-8)/组织途径金属蛋白酶抑制剂1(TIMP1)含量的影响。方法 92例慢性牙髓炎患者,随机分为对照组和观察组,每组46例。对照组采用根管手术后,给予常规药物治疗,观察组在对照组的基础上,加用速效牙痛宁酊辅助治疗。观察2组的临床效果,比较2组患者治疗前后龈沟液中白介素[白介素(IL)-1β、IL-8]、MMP-8/TIMP1的水平变化情况。结果观察组的总有效率显著高于对照组,差异有统计学意义(P<0.05);观察组疼痛发生率低于对照组,差异有统计学意义(P<0.05)。观察组龈沟液中IL-1β、IL-8及MMP-8/TIMP1的比例显著低于对照组,TIMP1的水平显著高于对照组,差异均有统计学意义(P<0.05)。结论速效牙痛宁酊配合常规疗法用于根管治疗后牙髓炎患者,可以有效调节龈沟液白介素、MMP-8/TIMP1水平,改善牙龈组织炎症,具有较好临床疗效、疼痛发生率低,值得推广和应用。

Objective To observe the effects of quick-impact toothache tincture combined with routine therapy on interleukin and MMP-8/TIMP1 levels in gingival crevicular fluid od patients with pulpitis after root canal therapy. Methods A total of 92 patients with pulpitis who were treated in our hospital from July 2014 to July 2016 were randomly divided into observation group（ n = 46） and control group（ n = 46）. The patients in control group were treated by routine medication after root canal surgery,however,the patients in observation group,on the basis of control group,were treated by quick-impact toothache tincture. The clinical effects,the changes of levels of IL-1β,IL-8 and and MMP-8/TIMP1 in gingival crevicular fluid before and after the treatments were observed and compared between the two groups. Results After treatment,the clinical total effective rate in observation group was significantly higher than that in control group（ P 〈 0. 05）,and the incidence rate of pain in observation group was 13. 0%,which was significantly lower than that（ 39. 1%） in control group（ P 〈 0. 05）. Moreover the levels of IL-1β,IL-8 and MMP-8/TIMP1 in observation group were significantly lower than those of control group,however,the levels of TIMP1 were significantly higher than those in control group（ P 〈 0. 05）. Conclusion The quick-impact toothache tincture combined with routine therapy can effectively regulate the levels of IL-1,IL-8 and MMP-8/TIMP1 in gingival crevicular fluid after root canal treatment so as to improve gum tissue inflammation,with satisfactory curative effects and less incidence of pain,therefor,which is worthy of clinical application and promotion.
