摘要
目的分析^(18)F-FDG PET/CT在胰头癌患者预后评估中的价值。方法回顾性分析65例胰头癌患者的临床及^(18)F-FDG PET/CT显像资料,测量胰腺原发灶最大标准摄取值(SUV_(max))、代谢肿瘤体积(MTV)和病灶糖酵解总量(TLG)。采用Cox比例风险模型获得胰腺癌患者预后的独立影响因素。结果65例胰头癌患者,原发灶SUV_(max)、MTV、TLG分别为7.9(5.6,10.4)、13.1(7.3,22.5)cm^3和68.6(32.2,95.7)。单因素生存分析结果显示,SUV_(max)、MTV、TLG、肿瘤最大径、淋巴结转移、肝转移、第8版美国癌症研究联合会分期、手术和化疗是胰头癌患者总生存期的影响因素(P均<0.05);多因素分析结果显示,手术、化疗、MTV、TLG是胰头癌患者总生存期的独立影响因素(P均<0.05)。结论 ^(18)F-FDG PET/CT指标MTV、TLG是影响胰头癌患者预后的独立预后因素,对评估胰头癌患者预后具有一定价值。
Objective To investigate the prognostic value of 18F-FDG PET/CT in patients with pancreatic head carcinoma.Methods The clinical and 18F-FDG PET/CT data of 65 patients with pancreatic head carcinoma were retrospectively analyzed.The maximum standardized uptake value(SUVmax),metabolic tumor volume(MTV)and total lesion glycolysis(TLG)of primary lesions were measured or calculated.The Coxproportional hazards model was used to obtain the independent prognostic influencing factors of pancreatic head carcinoma.Results SUVmax,MTV and TLG of 65patients with pancreatic head carcinoma was 7.9(5.6,10.4),13.1(7.3,22.5)cm3 and 68.6(32.2,95.7),respectively.Univariate analysis showed that SUVmax,MTV,TLG,the maximal tumor length,lymphadenectasis,hepatic metastases,American Jiont Committe on Cancer stage,operation and chemotherapy were all significantly associated with overall survival time(all P〈0.05).In multivariate analysis,the operation,chemotherapy,MTV and TLG were the independent influencing factors for overall survival time of patients with pancreatic head carcinoma(all P〈0.05).Conclusion MTV and TLG of 18F-FDG PET/CT are the independent prognostic factors for predicting prognosis of patients with pancreatic head carcinoma,which can provide certain value for prognosis of patients with pancreatic head carcinoma.
