80例输尿管结石输尿管镜下钬激光碎石的治疗效果

Effect of ureteroscopic holmium laser lithotripsy in 80 cases of ureteral calculi

目的观察输尿管镜下钬激光碎石术治疗输尿管结石的临床疗效。方法以在我院行输尿管镜下钬激光碎石术的80例输尿管结石患者为观察组,选择同期在我院行开放手术治疗的80例输尿管结石患者为对照组进行对照研究。所有患者收治起止时间为2015年1月至2017年12月。从手术时间、出血量、住院时间、并发症发生情况等指标比较两种方法的疗效,同时,比较两组患者术后胃肠功能恢复时间、下床活动时间、镇痛药物使用率。结果观察组患者手术时间、术中出血量、住院时间、并发症总发生率、术后镇痛药物使用率、术后胃肠功能恢复时间、下床活动时间均显著优于对照组,差异有统计学意义(P<0.05)。结论相较于传统的开放手术而言,输尿管镜下钬激光碎石术治疗输尿管结石的效果更好,安全性更高,对于患者术后恢复更有利,值得推广。

Objective To observe the clinical effect of ureteroscopic holmium laser lithotripsy in the treatment of ureteral calculi. Methods A total of 80 patients with ureteral calculi undergoing ureteroscopic holmium laser lithotripsy in our hospital were selected as bservation group, 80 patients with ureteral calculi undergoing open surgery in our hospital during the same period were selected as control group for a comparative study. The onset and stop time of all patients was from January 2015 to December 2017. The efficacy of the two methods was compared by comparing the operative time, bleeding volume, hospitalization time and complications between the two groups. At the same time, the recovery time of gastrointestinal function, ambulation time and utilization rate of analgesics were compared between the two groups after surgery. Results The operative time, intraoperative blood loss, hospitalization time, total incidence of complications, postoperative utilization rate of analgesics, postoperative gastrointestinal function recovery time and ambulation time in the observation group were significantly better than those in the control group, the difference were statistically significant（P〈0.05）. Conclusion Compared with the traditional open surgery, holmium laser lithotripsy under ureteroscope is better and safer for ureteral calculi, which is more favorable for postoperative recovery and is worthy of promotion.
