摘要
目的分析肝硬化合并肝功能衰竭的自身抗体与血浆炎症指标联合检测的诊断价值。方法选取2015年7月—2016年7月收治的肝硬化合并肝功能衰竭患者45例,并纳入观察组,另外选择相同时间内健康体检者45名,纳入对照组。分别测定两组的自身抗体与血浆炎症指标,将测定的结果作比较分析。结果观察组的血清PCT、CRP和IL-6水平分别为(1.92±0.34)μg/L、(14.21±1.63)mg/L和(0.75±0.05)ng/L,高于对照组的(0.21±0.05)μg/L、(7.02±1.03)mg/L和(0.31±0.02)ng/L,差异有统计学意义(P<0.05);对照组PCT、CPR以及IL-6阳性率分别为0.0%、2.2%和2.2%;显著低于观察组的88.9%、86.7%和82.2%,差异有统计学意义(P<0.05)。结论健康者的自身抗体可能存在阳性,在肝硬化合并肝功能衰竭的诊断中选择自身抗体与血浆炎症指标联合检测能更准确诊断,临床价值较高。
Objective The diagnostic value of combined detection of autoantibodies and plasma inflammatory markers in liver cirrhosis with liver failure was analyzed. Methods 45 patients with hepatic cirrhosis complicated with hepatic failure from July 2015 to July 2016 were included in the observation group. In addition, 45 healthy patients were selected and included in the control group at the same time. Two groups of autoantibodies and plasma inflammation were measured, and the results were compared and analyzed. Results Serum PCT, CRP, and IL-6 levels in the observation group were(1.92±0.34) μg/L,(14.21 ±1.63) mg/L, and(0.75±0.05) ng/L, respectively, which were higher than those in the control group(0.21±0.05) μg/L,(7.02±1.03) mg/L, and(0.31±0.02) ng/L, the difference was statistically significant(P〈0.05). PCT, CPR, and IL-6 positive rate in the control group were 0.0%, 2.2% and 2.2% respectively; significantly lower than the 88.9%, 86.7% and 82.2% of the observation group, the difference was statistically significant(P〈0.05).Conclusion There may be positive autoantibodies in healthy individuals. The combination of autoantibodies and plasma inflammatory markers in the diagnosis of liver cirrhosis combined with liver failure can be more accurately diagnosed and have higher clinical value.
