摘要
目的筛选胃食管反流病(GERD)患者呼出气体中差异性有机化合物,为研究其发病机制提供理论依据。方法2016年7月至2017年2月,用集气袋收集GERD患者及非GERD人群呼气样本59例,经固相微萃取法吸附后,用气相色谱/质谱对呼气中的挥发性有机化合物进行检测,筛选出GERD的挥发性标志物。结果采用正交信号校正-偏最小二乘法判别分析结果表明,GERD和非GERD人群呼出气体中存在着明显差异(模型参数为R2X[1]=0.286,R2X[2]=0.361),聚类明显(R^2=0.042,Q^2=-0.178);鉴定出邻苯二甲酸二乙酯(Diethyl Phthalate),邻苯二甲酸氢叔丁酯(1,2-Benzenedicarboxylic acid),吗啡林(Morpholine),三十二(碳)烷(Dotriacontane),十二烷(Dodecane),4-正十八烷基吗啡啉(4-Octadecyl-Morpholine)等6种差异代谢物(VIP=10.253 7、5.756 07、4.541 01、2.421 3、2.309 8、1.036 08,VIP>1)。结论 Diethyl Phthalate,1,2-Benzenedicarboxylic acid,Morpholine,Dotriacontane,Dodecane,4-Octadecyl-Morpholine等可能与GERD的发病有关,有待于进一步研究。
Objective To investigate the differentiation of organic compound in GERD patient's exhaled breath. Methods We collected a total of 59 cases of GERD and non-GERD patient's exhaled breath samples using gas-sampling bag, in order to screen volatile organic markers through solid phase microextraction and gaschromatography( GC)/mass spectrometry( MS) to test organic compound in exhaled breath samples. Results After orthogonal signal correction and partial least-squares discriminant analysis(PLS-DA),we found that there are differences between GERD and non-GERD patient's exhaled breath(model parameters are R2 X[1] = 0. 286 and R2 X[2] = 0. 361),clustering analysis showed obvious changes(R2 =0. 042 and Q2 =-0. 178). Finally,we detected different compounds expressed in two groups,such as Diethyl Phthalate, 1,2-Benzenedicarboxylic acid, Morpholine, Dotriacontane, Dodecane,4-Octadecyl-Morpholine( VIP =10.253 7、5.756 07、4.541 01、2.421 3、2.309 8、1.036 08.VIP 〉1). Conclusion Diethyl Phthalate, 1,2-Benzenedicarboxylic acid, Morpholine, Dotriacontane, Dodecane, 4-Octadecyl-Morpholine are probably associated with the pathogenesis of GERD, and our result may provide with theoretical basis for further clinical study.
