损伤控制外科手术治疗严重腹部创伤中的应用

Application of Damage Control Surgery in Treatment of Severe Abdominal Trauma

目的对损伤控制外科手术在严重腹部创伤治疗中的应用及效果进行研究。方法方便选取该院自2016年1月—2017年12月收治的42例严重腹部创伤急诊患者,其中23例按DCS原则处理,为DCS组,另19例非DCS组患者采取传统外科一期详尽手术治疗,术后于外科ICU中心进行常规复苏。比较两组患者的临床疗效及并发症发生率。结果观察组1例患者发生死亡,治疗总有效率为95.7%;对照组5例死亡,患者治疗总有效率为73.7%,两组对比差异有统计学意义(χ~2=6.257,P<0.05);此外,两组患者的手术时间(t=6.126,P<0.05)以及术中出血量(t=7.891,P<0.05)、住院时间(t=4.578,P<0.05)、术后并发症发生情况(χ~2=8.823,P<0.05)等疗效指标均差异有统计学意义。结论损伤控制外科手术进行严重腹部创伤患者治疗效果相对显著,且患者术后并发症相对较少,值得临床推广应用。

Objective To research the application and effect of damage control surgery in treatment of severe Abdominal trauma. Methods 42 cases of patients with severe abdominal trauma admitted and treated in our hospital from January 2016 to December 2017 were convenient selected and divided into two groups according to the DCS principle, including 23 cases in the DCS group and 19 cases in the non-DCS group, and given the routine recovery in the ICU center in the department of surgery, and the clinical curative effect and incidence rate of complications were compared between the two groups. Results 1 case died in the observation group, and the total effective rate was 95.7%, and 5 cases died in the control group,and the total incidence rate of treatment was 73.7%, and the difference was obvious（χ~2=6.257, P0.05）, with statistical significance, besides, the differences in the operation time, intraoperative bleeding amount, length of stay and postoperative complications between the two groups were big [（t=6.126, P0.05）,（t=7.891, P0.05）,（t=4.578, P0.05）,（χ~2=8.823, P0.05）].Conclusion The effect of damage control surgery in treatment of severe abdominal trauma is relatively obvious, with few postoperative complications, and it is worth clinical promotion and application.
