重复经颅磁刺激对卒中后抑郁患者睡眠质量的影响

Effect of Repetitive Transcranial Magnetic Stimulation on the Sleep Quality of Depression Patients after Stroke

目的探讨重复经颅磁刺激对脑卒中后抑郁患者睡眠质量的影响。方法方便收集2017年7—12月期间入福建省南平市人民医院接受诊治的52例脑卒中抑郁患者作为研究对象,将病例分为研究组(27例)和对照组(25例)。所有脑卒中抑郁患者均接受临床常规护理及治疗,研究组病例在此之上同时应用重复经颅磁刺激(rTMS)治疗,对比分析两组患者睡眠质量情况及抑郁情绪改善程度。结果治疗后研究组患者汉密顿抑郁量表评分(6.3±5.3)分明显低于对照组的(13.6±7.8)分(t=3.973,P=0.000);研究组8疗程后抑郁评分正常率(92.5%)显著高于对照组(40.0%)(χ~2=16.317,P<0.05)。治疗后研究组患者匹兹堡睡眠质量指数总评分(5.12±1.22)显著低于对照组(8.52±1.44)(t=9.209,P=0.000)。结论脑卒中后抑郁症患者在接受神经内科及抗抑郁治疗基础上,再行重复经颅磁刺激(rTMS)治疗可显著改善抑郁情绪和睡眠质量,提升患者生活质量。

Objective To study the effect of repetitive transcranial magnetic stimulation on the sleep quality of depression patients after stroke. Methods 52 cases of depression patients after stroke diagnosed in our hospital from July 2017 to December 2017 were convenient selected and divided into two groups, the research group with 27 cases and the control group with 25 cases received the clinical routine nursing and therapy, and the research group used the rTMS therapy on the basis of the above, and the sleep quality condition and improvement degree of depression emotion were compared between the two groups. Results After treatment, the Hamilton Depression Scale score in the research group was obviously lower than that in the control group, [（6.3±5.3）points vs（13.6±7.8）points],（ t=3.973, P=0.000）, and the normal rate of depression score after 8 treatment courses in the research group was obviously higher than that in the control group（92.5%vs 40.0%）,χ~2=16.317, P0.05）, and the total score of Pittsburgh sleep quality index in the research group after treatment was obviously lower than that in the control group[（5.12±1.22） vs（ 8.52±1.44）]（t=9.209,P=0.000）. Conclusion The rTMS treatment on the basis of the neurology and anti-depression therapy of depression patients after stroke can obviously improve the depression emotion and sleep quality and improve the quality of life.
