小儿麻醉维持中使用七氟醚与丙泊酚对患儿脑氧代谢影响

Effects of Sevoflurane and Propofol on cerebral oxygen metabolism in children's anesthesia maintenance

目的:探讨小儿麻醉维持中使用七氟醚与丙泊酚对患儿脑氧代谢影响。方法:选取88例接受外科手术治疗的患儿,分成两组,每组44例;对照组采用丙泊酚麻醉,观察组采用七氟醚麻醉;比较两组麻醉各个时刻血流动力学及脑氧量代谢指标。结果:t_1与t_4时刻的两组各脑氧量代谢指标对比,差异无统计学意义(P>0.05),t_2、t_3时,观察组Da-jvO_2、CERO_2指标低于对照组(P<0.05),观察组SjvO_2高于对照组(P<0.05)。与t_1相比,t_2、t_3及t_4时两组患儿MAP、HR值显著偏低(P<0.05);手术中各时刻两组患儿之间的MAP、HR值差异无统计学意义(P>0.05)。结论:小儿麻醉维持中使用七氟醚能降低脑氧量代谢的作用优于丙泊酚。

Objective： To study effects of Sevoflurane and Propofol on cerebral oxygen metabolism in children＇s anesthesia maintenance. Methods： 88 children underwent surgical treatment were selected and divided into the observation group（ 44 cases） and control group（ 44 cases）. The control group used Propofol anesthesia,while the observation group was given Sevoflurane anesthesia.The hemodynamics parameter at each time point during the anesthesia and cerebral oxygen metabolism indexes of the two groups were compared. Results： There were no significant differences in the t1 and t4 indexes between the two groups（ P〈0.05）. At t2 and t3,the Da-jvO2 and CERO2 of the observation group were lower than those of the control group（ P〈0.05）,and the SjvO2 was higher than that of the control group（ P〈0.05）. Compared with that at t1,the MAP and HR were significantly lower in the two groups at t2,t3,and t4（ P〈0.05）. There were no significant differences in MAP and HR between the two groups at each time point during the operation（ P〉0.05）. Conclusions： Sevoflurane is more effective than Propofol on cerebral oxygen metabolism in the children＇s anesthesia maintenance.
