老年髋部骨折采用手术治疗的预后及其影响因素

Prognosis of surgical treatment in elderly patients with hip fracture and its influencing factors

目的分析老年髋部骨折患者采用手术治疗的预后及其影响因素。方法将2015年9月至2017年6月于我院接受治疗的46例老年髋部骨折患者分为非手术组(n=20)和手术组(n=26)。非手术组采取保守治疗方式,手术组实施内固定手术或关节置换手术。比较两组并发症发生情况,分析手术治疗老年髋部骨折患者预后的影响因素。结果非手术组的并发症总发生率为50.00%,明显高于手术组的11.54%(P<0.05)。手术组老年髋关节患者预后的影响因素主要有年龄、术前基础疾病及并发症、手术方式及手术时机(P<0.05)。结论老年髋部骨折的临床治疗中,年龄、术前基础疾病及并发症数目、手术时间、手术类型等因素均会影响治疗效果,需要综合多种因素选择最佳的治疗方案。

Objective To analyze the prognosis of surgical treatment in elderly patients with hip fracture and its influencing factors.Methods Forty-six elderly patients with hip fracture admitted in our hospital from September 2015 to June 2017 were divided into non-surgery group（n=20） and surgery group（n=26）. The non-surgery group was treated with conservative treatment, and the surgery group was given internal fixation surgery or joint replacement surgery. The complications were compared between the two groups, and the influencing factors of prognosis in elderly patients who treated with surgical treatment were analyzed.Results The total incidence of complications in the non-surgery group was 50.00%, which was higher than 11.54% in the surgery group（P〈0.05）. The influencing factors of prognosis in elderly patients with hip fracture of the surgery group were age, preoperative underlying diseases and complications, surgical procedures and timing（P〈0.05）.Conclusion In the treatment of elderly patients with hip fracture, the age, the number of basic diseases and complications before operation, time of operation and type of operation all affect the treatment effect, so it is necessary to choose the best treatment plan by synthesizing many factors.
