磁共振扩散加权成像在肺癌中的临床应用

摘要 目的分析磁共振扩散加权成像(DWI)在肺癌中的应用效果。方法选取2012年1月至2017年6月我院收治的88例肺部疾病患者为研究对象,所有患者均进行DWI检查。分析肺部不同性质病灶的表面扩散系数(ADC)值与b值之间的关系,并对不同分化程度肺癌的ADC值进行比较。结果不同组织类型的肺癌及肺部炎性病变的ADC值与b值呈反比。b=300 s/mm^2时,肺癌患者中的腺癌及未分类癌患者ADC值与炎性病变患者的ADC值比较,差异具有统计学意义(P<0.05)。腺癌与鳞癌患者的ADC值比较,以及高分化肺癌与低分化肺癌患者的ADC值比较,差异均具有统计学意义(P<0.05)。结论 DWI在肺癌的鉴别诊断中具有积极的临床意义,可以将DWI中的ADC值作为鉴别肺部良恶性病变及肺癌不同组织类型的指标之一。 Objective To analyze the application effect of magnetic resonance diffusion-weighted imaging(DWI) in lung cancer.Methods From January 2012 to June 2017, 88 patients with lung disease were enrolled as the study objects.All the patients underwent DWI examination. The relationship between apparent diffusion coefficient(ADC) value and b value of different lung lesions was analyzed, and the ADC values of different differentiated lung cancer were compared.Results ADC values were inversely proportional to b values in lung cancer with different tissue types and pulmonary inflammatory lesions. When b =300 s/mm^2, the ADC values of adenocarcinoma and unclassified cancer patients were significantly different from those of inflammatory lesions(P〈0.05). There were significant differences in ADC values between adenocarcinoma and squamous cell carcinoma, and between well-differentiated lung cancer and poorly differentiated lung cancer(P〈0.05).Conclusion The differential diagnosis of lung cancer with DWI has a positive clinical significance,and the ADC value of DWI can be used as a differential diagnosis of benign lung lesions and lung cancer among different types of indicators.
