妇科恶性肿瘤患者的心理弹性、应对方式及生活质量相关性研究

Correlation between resilience,medical coping mode and quality of life in patients with gynecologic malignant tumor

目的探讨妇科三大恶性肿瘤患者的心理弹性、应对方式及生活质量的相关性。方法对76例妇科三大恶性肿瘤患者,采用一般资料调查表、心理弹性量表(CD-RISC)、医学应对方式问卷、癌症患者生命质量核心问卷(EORTC QLQ-C30)中文版进行调查。结果妇科癌症患者的心理弹性总分为(56.65±16.32)分,不同年龄、文化程度、经济状况患者心理弹性之间的差别有统计学意义(P<0.05);不同婚姻状况、费用支付方式及不同疾病类型患者心理弹性之间的差别无统计学意义(P>0.05);心理弹性总分与坚韧及自强维度、回避应对方式呈正相关(P<0.05),与面对及屈服应对方式呈负相关(P<0.05);CD-RISC总分及各维度得分与总体健康状况、认知功能等呈正相关(P<0.05),与疲倦、恶心呕吐等呈负相关(P<0.05)。结论医护人员要从认知及应对方式入手,重点加强对患者的认知行为干预,改变患者的主观认识,指导她们积极应对,促进身心健康。

Objective To investigate the relationship between the resilience,coping style and quality of life in patients with endometrial cancer,cervical cancer and ovarian cancer,and to provide scientific evidence for clinical nurses to develop nursing interventions and promote the relationship well. Methods The resilience scale（ CD-RISC）,medical coping mode questionnaire（ MCMQ）,cancer patients with the core quality of life questionnaire（ QLQ-C30） were used to survey in 76 cases of patients with gynecological malignant tumor. Results The CD-RISC of patients with gynecological malignant tumor was 56. 65 ± 16. 32,and there was different score of psychological flexibility in patients with different ages,different education and different nationalities（ P〈0. 05）. There was no different CD-RISC in patients with different marital status,payment method and disease type（ P〈0. 05）. The total score of CD-RISC was positively correlated toughness and self strength dimension and avoidance coping style（ P〈0. 05）,and avoidance coping style and coping style was negatively correlated with yield（ P〈0. 05）; the resilience of the score and overall health status,physical function,role function,emotional function,cognitive function and economic conditions were positively correlated（ P〈0. 05）,and psycho-logical resilience were negatively correlated fatigue,nausea and vomiting（ P〈0. 05）. Conclusion The medical staff should pay more attention to the psychological status of the patients,and encourage the patients to face adversity with positive emotion,so as to promote the physical and mental health development.
