小陷胸汤合桂枝茯苓丸治疗慢性阻塞性肺疾病急性加重期的临床研究

Clinical Observation of Xiaoxianxiong Decoction Combined with Guizhi Fuling Piull on Acute Exacerbation Chronic Obstructive Pulmonary Disease

目的观察小陷胸汤合桂枝茯苓丸治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床疗效并探讨其作用机制。方法 95例患者随机分为两组,均使用西医治疗,治疗组联合小陷胸汤合桂枝茯苓丸,比较两组治疗后的证候疗效,评价两组治疗前后的临床症状评分、CAT评分水平,对比两组治疗前后FEV1、FEV1占预计值的百分比、FEV1/FVC的变化。结果治疗组的总有效率95.83%及显效率70.83%均分别高于对照组的85.11%、46.81%(P<0.05或P<0.01)。治疗后两组临床症状评分、CAT评分较治疗前均下降(P<0.05),且治疗组较对照组下降更显著(P<0.05)。治疗后两组肺功能指标较前都显著改善(P<0.05),且与对照组相比,治疗组肺功能指标改善更显著(P<0.05)。结论小陷胸汤合桂枝茯苓丸药治疗AECOPD有良好的临床疗效,能改善AECOPD患者肺功能水平。

Objective： To study the clinical effect of Xiaoxianxiong Decoction combined with Guizhi Fuling Pill on acute exacerbation of chronic obstructive pulmonary disease（AECOPD）. Methods： 95 patients with AECOPD were randomly divided into the treatment group and the control group. Both groups were treated with Western medicine. The treatment group was added Xiaoxianxiong Decoction combined with Guizhi Fuling Pill. The following data was compared between the two groups： the curative effect after treatment,the clinical symptom scores before and after treatment,CAT scores,serum fibrinogen（FIB） and C-reactive protein（CRP）,and the percentage of FEV1 and FEV1 before and after treatment and the changes of FEV1/FVC in the two groups. Results： After treatment,the curative effect（including efficiency and obvious efficiency） of the treatment group was obviously better than that of the control group. The clinical symptom scores and CAT scores of the treatment group were obviously improved compared with the control group（P〈0.05）. The lung function of the treatment group was significantly improved after treatment（P〈0.05）,which was significantly different from that of the control group（P〈0.05）. Conclusion： Xiaoxianxiong Decoction combined with Guizhi Fuling Pill has a good clinical effect on AECOPD,and can improve the lung function of AECOPD patients.
