卡维地洛联合尼可地尔对冠心病心肌缺血患者心功能、并发症发生率及左室舒张功能的影响

Effect of Carvedilol Combined with Nicorandil on the Cardiac Function, Incidence Rate of Complications and Left Ventricular Diastolic Function

目的分析卡维地洛联合尼可地尔对冠心病心肌缺血患者的左室舒张功能、心功能和并发症发生率的影响。方法随机选择该院2016年8月—2017年9月收治的80例冠心病心肌缺血伴随左室舒张功能不全患者作为分析对象,均分成对照组(40例)和实验组(40例),对照组给予消心痛、拜阿司匹林进行常规治疗,实验组患者在对照组治疗基础上给予尼可地尔联合卡维地洛进行治疗。之后观察两组患者的左室舒张功能、心电图和NT-proBNP变化情况。结果两组心电图改善情况,实验组治疗后的ST段压低次数平均值(1.9±1.3)、心肌缺血总负荷(21.7±8.4)、持续时间下降(25.1±11.7)与对照组相比下降明显(P<0.05)。实验组的左室舒张功能E、IVSD、LVPWd、A、E/A等指标改善明显优于对照组(P<0.05);对照组治疗后的NT-proBNP是(496.2±86.8)ng/mL,实验组的是(325.2±100.2)ng/mL,差异有统计学意义(t=0.003,P<0.05)。结论对冠心病心肌缺血患者采取卡维地洛联合尼可地尔进行治疗临床效果显著,而且患者左室舒张功能改善明显。

Objective To analyze the effect of carvedilol combined with nicorandil on the cardiac function, incidence rate of complications and left ventricular diastolic function. Methods 80 cases of patients with myocardial ischemia of coronary heart disease with left ventricular diastolic function admitted and treated in our hospital from August 2016 to September 2017 were random selected and divided into two groups with 40 cases in each, the control group used the routine therapy,while the experimental group were treated with nicorandil and carvedilol, and the left ventricular diastolic function, electrocardiogram and NT-pro BNP changes of the two groups were observed. Results The average value of ST segment depression times, total ischemia burden and decrease of duration in the experimental group after treatment were respectively（1.9±1.3）,（21.7±8.4）,（25.1±11.7）,（P〈0.05）, and the improvement of E, IVSD, LVPWd, A, E/A in the experimental group were obviously better than that in the control group（P〈0.05）, and the NT-proBNP in the control group and in the experimental group after treatment was respectively（496.2±86.8）ng/mL and（325.2±100.2）ng/mL, and the difference was obvious,（t=0.003,P〈0.05）.Conclusion The clinical effect of carvedilol combined with nicorandil in treatment of patients with myocardial ischemia of coronary heart disease is obvious, and the improvement of left ventricular diastolic function is obvious.
