危重新生儿留置针脐静脉插管的应用研究

目的探讨在危重新生儿抢救中,使用留置针进行脐静脉插管,开放静脉液路。方法实验组选取2017年1—10月,在该院出生的危重新生儿,使用留置针进行脐静脉插管,建立静脉液路的患儿共28例。对照组选取2017年1—10月,分娩后立即转新生儿科住院的患儿,第一次建立静脉液路时,采用留置针外周静脉穿刺28例。比较留置针脐静脉插管与外周静脉留置针穿刺的一次穿刺成功率、开放静脉液路用时。结果实验组一次插管成功率89.29%,对照组一次插管成功率71.43%,两组数据比较差异有统计学意义(χ~2=15.05,P<0.01);实验组平均开放静脉液路用时为(2.33±1.18)min,对照组用时为(6.78±3.2)min,两组数据比较差异有统计学意义(t=7.017,P<0.01)。结论在危重新生儿抢救中,可以使用留置针进行脐静脉插管,以满足抢救时静脉用药的需求。

Objective To discuss the use of indwelling needle for umbilical vein intubation and open venous fluid pathway in the rescue of critically ill newborns. Methods From January 2017 to October 2017, a total of 28 cases of newborns in our hospital were treated with indwelling needles for umbilical vein intubation. The control group was selected from January to October 2017, and immediately transferred to the newborn pediatrics after delivery. In the first time, 28 cases of peripheral venous puncture were used for intravenous injection. The success rate of puncture and the time of opening venous fluid route for the puncture of intravenous catheter and peripheral vein catheter were compared. Result The success rate of onetime tube intubation in the experimental group and in the control group was respectively 89.29% and 71.43%, and the data of the two groups were significantly different（χ~2=15.05,P〈0.01）; The average time of opening venous fluid route in the experimental group and in the control group was respectively（2.33±1.18）min vs（6.78±3.2）min, and the data of the two groups were significantly different（t=7.017,P〈0.01）. Conclusion In the rescue of critically ill newborns, the indwelling needle can be used to intubate the umbilical vein in order to meet the need of the venous medication.
