强直性脊柱炎与HLA-B27的表达及T淋巴细胞亚群和细胞因子相关性分析

目的探讨强直性脊柱炎(AS)与HLA-B27、T淋巴细胞亚群及部分细胞因子之间的相关性,为AS的诊断、治疗及预后提供帮助。方法收集131例AS患者及50例健康人(健康对照组)外周血标本,采用流式细胞术检测外周血中T淋巴细胞亚群及HLA-B27的表达情况,固相夹心法化学发光免疫量度法对血清IL-1β、IL-6、IL-8、TNF-α等细胞因子进行定量检测。结果 AS患者与健康对照组比较,CD3+CD4-CD8+T(T8细胞)、IL-8、TNF-α升高,差异有统计学意义(P<0.05),CD3+T(总T细胞)、CD3+CD4+CD8-T(T4细胞)、T4/T8、IL-1β、IL-6差异无统计学意义(P>0.05)。HLA-B27阳性AS患者和HLA-B27阴性AS患者比较,T淋巴细胞亚群和上述细胞因子间均无显著差异(P>0.05)。AS患者HLA-B27不同荧光强度组别间上述各指标均无显著差异(P>0.05)。结论 HLA-B27、T淋巴细胞亚群和细胞因子IL-8、TNF-α的检测有助于提高AS诊断敏感性,对判断AS病情的发展转归有一定价值,HLA-B27的荧光表达强弱可能不反映AS病情进展情况。

Objective To study the correlation of the HLA-B27 and T lymphocytes subsets and some cytokines of the ankylosing spondylitis（AS）patients,so as to help the diagnosis,treatment and prognosis of AS.Methods In the study 50 healthy controls and 131 patients with ankylosing spondylitis were selected as the research objects.The peripheral blood samples of 131 AS patients and 50 healthy people（healthy control group）were collected.Flow cytometry was used to detect the expression of T lymphocyte subsets and HLAB27 in peripheral blood.The serum IL-1β,IL-6,IL-8 and TNF-αwere measured by solid phase sandwich chemiluminescence immunoassay.Results Compared with the healthy control group,the CD3^＋CD4^-CD8^＋T（T8 cell）,IL-8 and TNF-α were increased in AS patients,and the difference was statistically significant（P〉0.05）,CD3^＋T（total T cells）,CD3^＋CD4^＋CD8^-T cells（T4 cell）,T4/T8,IL-1βand IL-6 had no statistical significance（P〈0.05）.There was no significant difference in T lymphocyte subsets and cytokines between HLA-B27 positive AS patients and HLA-B27 negative AS patients（P〉0.05）.There was no significant difference in the above indexes among AS patients with different HLA-B27 fluorescence intensities（P〉0.05）.Conclusion The detection of HLA-B27,T lymphocyte subsets and cytokine IL-8,TNF-αcan help to improve the sensitivity of AS diagnosis,and have some value to assess the prognosis of AS,and the fluorescence intensity of HLA-B27 may not reflect the progression of AS.
