关节镜下膝关节腔内引流联合囊肿壁刨除术治疗腘窝囊肿的临床研究

Arthroscopic treatment of popliteal cyst with arthroscopic intracavitary drainage combined with cystectomy

目的研究关节镜下膝关节腔内引流联合囊肿壁刨除术对腘窝囊肿的治疗效果。方法选择我院2017年4月—2018年4月收治的60例腘窝囊肿患者作为研究对象,随机将其分为对照组和观察组,每组30例。其中对照组患者进行膝关节腔内引流术治疗,观察组患者则进行膝关节腔内引流术与囊肿壁刨除术联合治疗,比较2组患者的治疗效果。结果 2组患者手术时间、手术切口、术中流血量、住院时间等手术一般情况差异均不明显(P>0.05);2组患者手术前的Lysholm膝关节功能评分相近,手术后观察组评分显著高于对照组(P<0.05);观察组治疗优良率为100.00%,显著高于对照组的83.33%(P<0.05);2组患者术后3个月、6个月腘窝囊肿的复发率无明显差异,但术后12个月观察组患者的复发率为10.00%,显著低于对照组的36.67%(P<0.05)。结论关节镜下膝关节腔内引流联合囊肿壁刨除术治疗腘窝囊肿的效果良好,可显著改善患者的膝关节功能,降低复发率,值得推广应用。

Objective To study the effect of arthroscopic intracavitary drainage combined with cystectomy for popliteal cyst.Methods 60 patients with popliteal cysts admitted to our hospital from April 2017 to April 2018 were selected as study subjects.They were randomly divided into control group and observation group with 30 cases in each group.The patients in the control group were treated with intracavitary drainage of the knee joint.The patients in the observation group were treated with intraluminal drainage of knee joint and cystectomy.The therapeutic effects of the two groups were compared.Results There were no significant differences in the operation time,incision,bleeding and hospital stay between the two groups（P〉0.05）.There was no significant difference in Lysholm knee function score between the two groups before operation,but after operation,the Lysholm score of knee joint in the observation group was significantly higher than the control group（P〈0.05）.In the observation group,there were 12 cases with excellent therapeutic effect,accounting for 40.00 g.The excellent and good rate of treatment was 100.00%,which was significantly higher than the control group（P〈0.05）.The recurrence rate of popliteal cyst was not significantly different between the two groups at 3 and 6 months postoperatively,but 12 months after operation,the recurrence rate of the patients in the observation group was 10.00%,which was significantly lower than that in the control group（36.67%）（P〈0.05）.Conclusion Arthroscopic intracavitary drainage combined with cystectomy is effective in the treatment of popliteal cysts.It can significantly improve the knee joint function and reduce the recurrence rate.It can be widely recommended in clinical practice.
