院外癌症疼痛患者的疼痛现状调查及用药行为影响因素分析

Investigation of the pain situation and influencing factors of medication behavior in outpatients with cancer pain

目的研究癌症患者在院外的疼痛程度、疼痛原因,并分析其用药行为的影响因素。方法选择在我院就诊的具有不同疼痛程度的癌症患者,共268例,采用标准化的疼痛量表(BPI-C)及问卷调查形式,调查患者出院后的疼痛现状及其影响因素。结果 268例院外随访患者中,过去1周有95.9%的患者发生不同程度的疼痛,其中轻度、中度、重度疼痛患者分别占22.0%、32.1%、41.8%,患者的疼痛受到伴随症状、功能状态、文化程度等因素影响。只有88%的患者能遵医嘱用药,其余12%患者未能遵医嘱用药,患者遵医嘱用药行为主要受到文化程度、医疗支付方式、伴随症状(便秘、恶心呕吐、眩晕等)等因素影响。结论院外患者的疼痛现状比较严重,社会人员特别是医务工作者应更多关注患者的院外疼痛,为患者在院外提供更多的医疗指导,以帮助患者减轻疼痛,改善生活质量。

Objective To investigate the degree and causes of pain in outpatients with cancer,and analyze the influencing factors of medication behavior. Methods A total of 268 cancer patients with different degree of pain in our hospital were selected and a standardized pain scale （BPI-C） and questionnaire survey were used to investigate the sta- tus of pain and its influencing factors in patients after being discharged from hospital. Results In the past week, there were 95.9% of the outpatients with different degrees of pain,of which,patients with mild pain,moderate pain and se- vere pain counted for 22% ,32. 1% and 41.8% ,respectively. The patient＇s pain was affected by symptoms, functional status, educational degree, etc. Only 88% of the outpatients were able to follow the doctor＇s advice, the remaining 12% patients failed to follow the doctor＇s advice, which was influenced mainly by educational degree, mode of medical payment,and accompanied symptoms （ e. g, constipation, nausea and vomiting, vertigo, etc. ）. Conclusion The present situation of pain in outpatients with cancer is serious;the social personnel, especially the medical staff should pay more attention to the pain of outpatients and give medical guidance to help relieve the pain and improve the quality of life.
