伴耳鸣的突聋患者的耳鸣治疗影响因素分析

Analysis of Influencing Factors on the Clinical Efficacy of Tinnitus in Patients with Sudden Deafness and Tinnitus

目的分析突发性聋伴耳鸣患者的耳鸣临床疗效影响因素。方法回顾性分析2012年1月至2017年6月到我院治疗且诊断为突发性聋伴耳鸣76例(82耳)患者的资料,统计患者年龄、耳别、听力损失程度、听力曲线类型、听力损失疗效,并按照一定标准进行分组,比较出这6大因素对耳鸣疗效的影响。结果耳鸣疗效在不同年龄段与不同耳别患者之间差异无统计学意义(P>0.05);耳鸣疗效与听力损失程度有相关性(P<0.05),极重度聋患者的耳鸣总有效率明显低于其他各程度听力损失(P<0.05);耳鸣疗效与听力曲线有相关性(P<0.05),听力曲线为全聋型患者的耳鸣总有效率明显低于其他各听力曲线类型者(P<0.05);听力损失疗效为无效者明显低于听力损失疗效为痊愈、显效、有效者耳鸣总有效率(P<0.05),但其余听力损失疗效为痊愈、显效、有效分级之间的耳鸣疗效比较差异无统计学意义(P>0.05)。结论突发性聋伴耳鸣患者听力损失疗效较耳鸣疗效差;耳鸣疗效与不同耳别、各年龄段无关,与听力损失程度、听力曲线类型、听力损失疗效有关;极重度听力损失、听力曲线为全聋型患者、听力损失治疗无效者的耳鸣疗效差。

Objective To analyze the influencing factors on the clinical efficacy of tinnitus in patients with sudden deafness and tinnitus. Methods 76 cases（ 82 ears） with sudden deafness and tinnitus in out hospital from January 2012 to June 2017 were retrospectively analyzed. Age,ear type,degree of hearing loss,type of hearing curve and curative effect of hearing loss were statistically analyzed. Groups were divided according to certain criteria and the effects of these six factors on tinnitus were compared. Results There was no significant difference in the therapeutic effect among patients with different ages and different ear type（ P〈0. 05）. Therapeutic effect was correlated with the degree of hearing loss（ P〈0. 05）. Total efficiency of severe deafness was significantly lower than other degrees of hearing loss（ P〈0. 05）. Therapeutic effect was correlated with the hearing curve（ P〈0. 05）. Total tinnitus curative efficiency of complete deafness patients was significantly lower than other types in audiometric curve（ P〈0. 05）. Total effective rate of tinnitus in patients with ineffective treatment in hearing loss was significantly lower than those with curative effect of hearing loss being cured,effective and effective. However,there was no significant difference in tinnitus efficacy in patients of other hearing loss with recovery,remarkable effect and effective grading（ P〈0. 05）. Conclusions The effect of hearing loss in patients with sudden deafness and tinnitus is worse than that of tinnitus. The curative effect of tinnitus is not related to different ear types and ages,but to degree of hearing loss,type of hearing curve and curative effect of hearing loss. The curative effect of tinnitus is poor in patients with extremely severe hearing loss,hearing curve being of total deafness and hearing loss being ineffective.
