以循证理念为基础的疼痛护理对胆囊结石腹腔镜术后患者的影响

Application of evidence-based pain management in patients with gallbladder laparoscopy

目的:探讨以循证理念为基础的疼痛护理对胆囊结石腹腔镜术后患者的影响。方法:将2017年6月~2018年6月收治的90例行腹腔镜治疗的胆囊结石患者随机分为研究组和对照组各45例,对照组给予常规护理,研究组实施以循证理念为基础的疼痛护理。采用视觉模拟评分法(VAS)评价两组护理前后疼痛程度,记录两组疼痛发生情况、持续时间,采用健康调查简表(SF-36)评价两组护理前后生活质量。结果:两组护理后VAS评分较护理前均明显降低(P<0.05),且研究组低于对照组(P<0.05);研究组肩膀酸痛及牵涉疼痛发生率低于对照组,疼痛持续时间短于对照组(P<0.05);两组护理后SF-36各项指标评分较护理前均明显升高(P<0.05),且研究组高于对照组(P<0.05)。结论:对行腹腔镜治疗的胆囊结石患者实施以循证理念为基础的疼痛护理,可明显缓解患者疼痛程度,提高其生活质量,效果显著,具有较高的临床应用价值。

Objective： To explore the effect of pain nursing based on evidence-based concept in patients with cholecystolithiasis after laparoscopic surgery. Methods： A total of ninety patients undergoing laparoscopic treatment of gallstones from June 2017 to June 2018 were randomly divided into study group and control group,45 cases in each group. The control group received routine care and the study group implemented pain-based care based on evidence-based concepts. The visual analog scale（ VAS） was used to evaluate the pain level before and after treatment. The pain occurrence and duration of the two groups were recorded. The health survey summary（ SF-36） was used to evaluate the quality of life before and after treatment. Results： The VAS scores of the two groups were significantly lower（ P〈0. 05）,and the study group was lower than the control group（ P〈0. 05）. The incidence of shoulder pain and pain involved in the study group was lower than that of the control group,and the pain duration was shorter than that of the control group（ P〈0. 05）; the scores of SF-36 indexes were significantly increased after treatment（ P〈0. 05）,and the study group was higher than the control group（ P〈0. 05）. Conclusion： For patients with cholecystolithiasis treated by laparoscopy,the pain nursing based on evidence-based concept can obviously relieve the pain degree and improve the quality of life. The clinical effect is remarkable,and it has high clinical value.
