19例腰椎手术失败医疗纠纷案的探讨

Study on 19 Medical Disputes Caused by Lumbar Spine Operation Failure

目的目的:探讨腰椎手术失败医疗纠纷案的特点及导致失败的原因,为防范该失败及其医疗纠纷鉴定实践提供参考。方法:回顾性分析对19例腰椎手术失败引发医疗纠纷案,归纳其基本情况、手术失败的原因、因果关系认定及各级医院责任程度分布。结果:发病患者多为中老年;男性多于女性;多数患者椎间盘突出及椎管狭窄并存;医院的专业技术水平高低与医院级别存在一定关系。结论:提高医务人员的专业技术水平、严格掌握手术适应症、完善风险告知制度、保证良好的医疗服务态度是有效预防此类医疗纠纷发生的有效途径;掌握正确的司法鉴定方法是处理此类医疗纠纷的重要保障。

Objective： To explore the characteristicsof medical dispute of lumbar spine operation failure andits causes, and provide practical reference for preventing the failure and its medical dispute appraisal. Methods： A retrospective analysis of 19 medical disputes caused by the failure of lumbar spine operation failure was conducted to conclude its basic situation, failure causes, the determination of the cause and effect and the distribution of responsibilities of hospitals at all levels. Results： Most of the patients were middle-aged and elderly;male patients are more than femalae patients; herniated disc and stenosiscoexisted in most of the patients;and there is a certain relation betweenthe level of expertise and the level of hospital. Conclusion： It is effective prevent the occurrence of such medical disputes by improving the professional technical level of medical personnel, strictly mastering the surgical indications,improving risk information system,and ensuring good medical service. And mastering the correct method of judicial appraisal is an important guarantee to deal with such medical disputes.
