实施基本公共卫生服务项目在糖尿病管理中的效果评估

Effect evaluation of implementing basic public health services in diabetes management

目的:探讨实施基本公共卫生服务项目在糖尿病管理中的效果。方法:抽取本省市3个街道及周边地区的4个乡镇,至少涉及11家社区卫生服务中心,本次调查总共3300位常住人口,有效样表3270份,对糖尿病患者当前病况、用药、治疗及术后并发症情况加以询问,并给予患者血糖、血压测量,总结糖尿病的危险因素,计算患者的知晓率、治愈率、控制率。结果:通过调查结果显示,18岁年龄阶段的糖尿病患病率达6.27%,大于35岁年龄阶段的糖尿病患病率达7.96%,且糖尿病发病率与年龄呈现正相关。将以上诸多可疑因素进行糖尿病Logistic回归后,确定年龄、职业、婚姻状况、腰围长、收缩压、饮食习惯、家族史、睡眠质量为糖尿病危险因素,文化程度为保护因素。在调查现场中,经统计总共有154例主诉糖尿病患者,疾病知晓率为75.12%;在已知患者中,存在127例服药患者,治愈率为61.59%;69例血糖指标康复者,控制率为33.66%。结论:通过实施基本公共卫生服务项目,在降低治疗成本与负担下,民众糖尿病知晓率、治愈率以及控制率得到显著提升,同时也能全面统计当下影响居民患糖尿病的诸多因素,及时、有效为糖尿病患者制定更好的预防措施与制度,造福社会与广大民众。

Objective：To explore the effect of implementing basic public health services in diabetes management.Methods：4 villages and towns in 3 streets and surrounding areas of the province were selected,and at least 11 community health service centers were involved.A total of 3300 permanent residents were investigated and 3270 valid samples were collected.The patients＇ current conditions,medication,treatment and postoperative complications were asked,and blood glucose and blood pressure were given to the patients.The risk factors of diabetes were summarized,and the awareness rate,cure rate and control rate of patients were calculated.Results：The results of the survey showed that the prevalence of diabetes at the age of 18 was 6.27%,and the prevalence of diabetes was 7.96% over the age of 35,and the incidence of diabetes was positively correlated with age.After the Logistic regression of many suspected factors,age,occupation,marital status,waist length,systolic pressure,diet habit,family history and sleep quality were the risk factors of diabetes,and the degree of cultural protection was the protection factor.In the survey site,there were 154 cases of diabetic patients who complained of diabetes,and the awareness rate of disease was 75.12%.Among the known patients,there were 127 cases of medication,the cure rate was 61.59%,and69 cases of blood glucose index recovered,the control rate was 33.66%.Conclusion：Through the implementation of the basic public health service projects,the awareness rate,cure rate and control rate of diabetes in the people have been significantly improved in reducing the cost and burden of treatment.At the same time,it can also make a comprehensive statistics on the factors affecting the diabetes in the residents,and make better preventive measures for the patients with diabetes mellitus in time and effectively.The system will benefit the society and the masses.
