第四代陶瓷界面髋关节假体在全髋关节置换术中的应用效果观察

The clinical outcomes of fourth-generation ceramic-on-ceramic total hip arthroplasty

目的:观察第四代陶瓷界面髋关节假体在全髓关节置换术中的应用效果。方法:回顾性分析2015年1月至2016年12月在本院行全髋关节置换术的84例(92髋)患者的临床资料,所有患者均应用第四代陶瓷界面髋关节假体。术后对患者进行平均为期12个月的随访,观察临床治疗效果。结果:84例患者平均随访时间为(12.37±1.38)个月,其中3例患者在术后发生下肢静脉血栓,1例患者术后发生外伤性股骨假体周围骨折,经有效治疗后治愈出院,另有1例患者外展患肢时出现异常声响。所有患者术后均无明显感染、脱位等。假体未出现松动、破碎或者骨溶解等并发症。患者术后髋关节活动评分较术前明显改善,平均Harris评分由术前的(41.65±3.38)分升高至术后的(86.17±3.72)分(P<0.05)。髋关节Harris评分:优33例(36髋),良46例(50髋),可4例(5髋),差1例(1髋)。结论:在全髋关节置换术中应用第四代陶瓷—陶瓷界面,能够有效改善患者术后髋关节活动水平,短期治疗效果良好。

Objective：To observe the clinical results and complications of fourth-generation ceramic-on-ceramic total hip arthroplasty（THA）.Methods：The clinical data of 84 patients（92 hips）who underwent THA with the fourth generation of ceramic-on-ceramic total hip prosthesis in our hospital from January2015 to December 2016 were retrospective analyzed.The clinical outcomes and complications were observed after 12 months of surgery.Results：The mean follow-up duration was 12.37±1.38 months.There were3 cases with lower extremity deep venous thrombosis and 1 cases with traumatic femoral periprosthetic fracture at post-operation.After effective treatment,the patients were cured and discharged from the hospital.One affected limb exhibited audible noise when abduction.No obvious postoperative complications such as infection,dislocation,osteolysis and prosthesis loosening were found in all cases.The Harris hip score was significantly elevated after operation（86.17±3.72 vs.41.65±3.38）（P〈0.05）.Harris scores were excellent in 36 hips,good in 50,fair in 5 and poor in only 1.Conclusion：The fourth-generation ceramic-onceramic THA demonstrated good clinical results at a short-term follow-up study.
