脑卒中吞咽困难识别与管理循证实践项目持续性影响因素的质性研究

A qualitative study on factors influencing the sustainability of an evidence-based nursing innovation program-dysphagia identification and management in post-stroke patients

目的探讨影响脑卒中吞咽困难识别与管理循证实践项目持续性的因素,为制订相应的行动策略推动项目持续提供依据。方法采用质性研究中现象学的研究方法,对9名循证实践项目的利益相关者进行半结构深入访谈,采用Colaizzi现象学分析法对获取的资料进行分析。结果影响该循证实践项目持续的因素包括3个方面,分别为项目特点:必要性、首优性和完整性;组织环境:人力资源、培训与宣传、基础设施;合作意识:护患配合度、医护配合度和科研与临床配合度。结论循证实践项目的临床价值,组织对项目持续提供的支持性环境和跨专业团队之间的合作程度等是影响该项目持续的重要因素。

Objective To explore the factors influencing the sustainability of an evidence-based nursing innovation program-dysphagia identification and management in post-stroke patients,and to provide evidence basis for the development of appropriate action strategies to promote program sustainability.Methods Semi-structured in-depth interviews were conducted to 9 program stakeholders.Data were analyzed with Colaizzi phenomenological analysis method.Results The factors influencing program sustainability include 3 themes by phenomenological method program characteristics include necessity,priority and completeness,organizational environment include human resource,training and publicity and infrastructure,sense of collaboration include nurse-patient collaboration,nurse-doctor collaboration and clinician-researcher collaboration.Conclusion Clinical value of the evidence implementation project,supporting environment provided by the organization and degree of cooperation among the interdisciplinary team are important factors impacting the sustainability of evidence-based practice programs.
