瑞芬太尼复合丙泊酚靶控输注麻醉在妇科腹腔镜手术中的效果比较

Comparison of Remifentanil and Propofol Targeted Infusion Anesthesia in Gynecologic Laparoscopic Surgery

目的探讨妇科腹腔镜手术中应用瑞芬太复合丙泊酚靶控输注麻醉的临床效果。方法随机选取2016年1月—2018年2月该院妇科100例腹腔镜手术患者为研究对象,依据麻醉方法不同分为实验组(靶控输注维持麻醉,50例)和对照组(恒速泵控维持麻醉),对比观察两组麻醉效果及基本麻醉指标。结果实验组麻醉优良率(96.0%)与对照组(90.0%)比较差异无统计学意义(χ~2=2.67,P>0.05);实验组术中心率、血压较对照组平稳,术后睁眼时间(6.1±1.7)min,拔管时间(11.6±2.4)min,定向力恢复时间(18.3±5.1)min,与对照组比较差异有统计学意义(t=5.13,P<0.05;t=5.85,P<0.05;t=6.07,P<0.05)。结论妇科腹腔镜手术中应用瑞芬太尼复合丙泊酚靶控输注维持麻醉效果确切,患者术中血流动力学平稳,术后恢复快,安全性高,值得推广使用。

Objective This paper tries to investigate the clinical effect of remifentanil in the application of propofol in gynecologic laparoscopic surgery. Methods 100 patients undergoing laparoscopic surgery in the hospital from January 2016 to February 2018 were random selected as the research object, according to different anesthesia methods, they were divided into experimental group（target controlled infusion maintenance anesthesia, 50 cases） and control group（constant speed pump control anesthesia）, and the anesthetic effect and basic anesthesia index were compared between the two groups. Results The rate of anesthesia in the experimental group（96.0%） was not statistically different from that in the control group（90.0%）（χ~2=2.67, P〉0.05）; the center rate and blood pressure of the experimental group were more stable than those of the control group, and the eye opening time after surgery（6.1±1.7）min, extubation time（11.6 ± 2.4）min, orientation recovery time（18.3±5.1）min, statistically significant difference from the control group（t=5.13, P〈0.05; t=5.85, P〈0.05; t=6.07, P〈0.05）.Conclusion In gynecologic laparoscopic surgery, remifentanil combined with propofol target-controlled infusion maintains anesthetic effect. The patient＇s intraoperative hemodynamic stability, rapid postoperative recovery, and high safety are worth promoting.
