cTnI联合NT-proBNP检测对老年重症心力衰竭患者的临床价值

Clinical Value of cTnI Combined with NT-proBNP in Elderly Patients with Severe Heart Failure

目的探讨心肌肌钙蛋白I(cTnI)联合N末端B型钠尿肽原(NT-proBNP)诊断老年重症心力衰竭的临床价值。方法方便选取2014年1月—2017年10月该院40例老年重症心力衰竭患者为实验组,取同期40例老年非重症心力衰竭患者为对照1组,取同期40名老年健康志愿者为对照2组。测定3组cTnI、NT-proBNP水平,观察实验组不同预后患者cTnI、NT-proBNP水平变化。结果实验组cTnI(0.74±0.20)μg/L,NT-proBNP(7 536.4±2 259.6)ng/L,与对照1组、2组比较差异有统计学意义(t=3.82,7.56,77.94,91.37,P<0.05),实验组生存患者出院时cTnI(0.11±0.03)μg/L,NTproBNP(1 582.4±613.4)ng/L,与死亡患者比较,差异有统计学意义(t=4.81,87.61,P<0.05)。结论 cTnI联合NT-proBNP检测对老年重症心力衰竭临床诊断及预后评估具有指导意义,值得推广使用。

Objective This paper tries to investigate the clinical value of cardiac troponin I（cTnI） combined with N terminal B type natriuretic peptide（NT-proBNP） in the diagnosis of severe heart failure in the elderly. Methods From January2014 to October 2017, 40 patients with severe heart failure in the hospital were included conveniently in the experimental group. 40 patients with non-critical heart failure in the same period were taken as control group one, and 40 healthy volunteers in the same period were used as control group two. The levels of cTnI and NT-proBNP in the three groups were measured. The changes of cTnI and NT-proBNP levels in the experimental group were observed. Results The experimental group cTnI（0.74±0.20）μg/L, NT-proBNP（7 536.4±2 259.6）ng/L, compared with the control group 1, 2 groups the difference were statistically significant（t =3.82,7.56,77.94,91.37,P〈0.05）. Survival patients in the experimental group were discharged with cTnI（0.11 ±0.03）μg/L and NT-proBNP（1 582.4 ±613.4）ng/L. There was a statistically significant difference compared with those who died（t=4.81,87.61, P〈0.05）. Conclusion The detection of cTnI combined with NT-proBNP has important significance in the clinical diagnosis and prognosis evaluation of severe heart failure in the elderly. It is worthy of promotion.
