不同糖耐量的中老年男性患者T淋巴细胞亚群变化

摘要 目的观察不同糖耐量的中老年男性患者T淋巴细胞亚群变化情况。方法选取2013年12月至2018年1月在安徽省立医院老年科住院患者及体检的124例中老年男性为研究对象,其中糖耐量正常组41例、糖耐量减低组40例、2型糖尿病(T2DM组)43例。检测各组受试者体质指数(BMI)、收缩压、舒张压、餐后2 h血糖(2 h PBG)、糖化血红蛋白(Hb A_(1c))、三酰甘油(TG)、总胆固醇和T细胞亚群(CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+),采用稳态模型评估的胰岛素抵抗指数(HOMA-IR)评估机体的胰岛素抵抗。结果各组BMI、2 h PBG、Hb A_(1c)、TG、HOMA-IR比较差异有统计学意义(P<0.05)。各组CD_3^+、CD_4^+、CD_8^+和CD_4^+/CD_8^+比较差异有统计学意义(P<0.01),糖耐量减低组和T2DM组CD_3^+、CD_4^+和CD_4^+/CD_8^+低于糖耐量正常组,T2DM组低于糖耐量减低组(P<0.05);糖耐量减低组和T2DM组CD_8^+高于糖耐量正常组,T2DM组高于糖耐量减低组(P<0.05)。结论糖代谢异常的中老年男性存在T淋巴细胞亚群比例失衡。 Objective To observe the changes of T lymphocyte subsets level in middle-aged and elderly male patients with different glucose tolerance. Methods From Dec. 2013 to Jan. 2018,124 middle-aged and elderly male who were hospitalized and underwent health examination in Anhui Provincial Hospital were included in the study: including 41 patients with normal glucose tolerance,40 patients with reduced glucose tolerance and 43 patients with type 2 diabetes( T2 DM).Body mass index( BMI),systolic blood pressure( SBP),diastolic blood pressure( DBP),two hours postprandial blood glucose( 2 h PBG),glycosylated hemoglobin( Hb A1 c),triglyceride( TG),total cholesterol( TC),T cell subsets( CD3~+,CD4~+,CD8~+,CD4~+/CD8~+ ratio) were measured. Homeostasis model assessment insulin resistant index( HOMA-IR) was used to evaluate the body's resistance to insulin. Results The differences of BMI,2 h PBG,Hb A1 c,TG,HOMA-IR in each group were statistically significant( P〈0. 05). The differences of CD3~+,CD4~+,CD8~+,CD4~+/CD8~+ ratio in each group were statistically significant( P〈0. 05),the T2 DM group and reduced glucose tolerance group had significantly lower levels of CD3~+,CD4~+,CD4~+/CD8~+ ratio compared with the normal glucose tolerance group,and that in the T2 DM group was lower than the reduced glucose tolerance group( P〈0. 05). The T2 DM group and reduced glucose tolerance group had significantly higher levels of CD8~+ compared with the normal glucose tolerance group,while that in the T2 DM group was higher than the reduced glucose tolerance group( P〈0. 05). Conclusion T lymphocyte subsets are imbalanced in middle-aged and elderly men with abnormal metabolism of glucose.
