健脾利湿通络法治疗2型糖尿病合并脂代谢异常的临床疗效

Analysis of Efficacy of Invigorating Spleen and Removing Dampness and Dredging Collaterals in the Treatment of Type 2 Diabetes Combined with Abnormal Lipid Metabolism

目的探讨健脾利湿通络法治疗2型糖尿病合并脂代谢异常的临床疗效。方法选取2015年8月至2017年8月武汉市普仁医院内分泌科收治的120例2型糖尿病合并脂代谢异常患者为研究对象,按照治疗方法不同分为对照组和观察组,各60例。对照组采用饮食控制、运动疗法、常规降糖降脂药物治疗等常规治疗,观察组在对照组治疗的基础上运用健脾利湿通络法治疗,两组患者疗程均为16周。观察两组患者的临床疗效以及治疗前后血糖[空腹血糖(FBG)、糖化血红蛋白(Hb A1c)、餐后2 h血糖(2 h PBG)]及血脂[总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]水平。结果观察组总有效率高于对照组[91.67%(55/60)比78.33%(47/60)](P<0.05)。治疗后,观察组FBG、Hb A1c、2 h PBG低于对照组[(5.22±0.39)mmol/L比(5.68±1.61)mmol/L,(5.08±1.04)mmol/L比(5.49±1.03)mmol/L,(7.78±0.89)mmol/L比(8.13±0.84)mmol/L](P<0.05)。治疗后,观察组TC、TG、LDL-C低于对照组[(3.96±0.35)mmol/L比(4.59±0.56)mmol/L,(1.63±0.58)mmol/L比(2.06±0.65)mmol/L,(2.68±0.97)mmol/L比(3.17±0.63)mmol/L](P<0.01),HDL-C高于对照组[(1.98±0.57)mmol/L比(1.79±0.46)mmol/L](P<0.05)。结论健脾利湿通络法联合常规治疗2型糖尿病合并脂代谢异常有较好的临床疗效,在改善血脂、血糖等方面均优于单独常规西医治疗。

Objective To investigate the clinical efficacy of invigorating spleen and removing dampness and dredging collaterals treatment for type 2 diabetic patients with abnormal lipid metabolism. Methods From Aug. 2015 to Aug. 2017,120 type 2 diabetic patients with abnormal lipid metabolism admitted to the Department of Endocrinology,Wuhan Puren Hospital were divided into a control group and an observation group（ 60 cases each） according to the different treatment methods.The control group took the diet control,exercise therapy,drug therapy for the treatment of diabetes. The observation group was given invigorating spleen and removing dampness and dredging collaterals treatment based on the control group＇ s regimen.The course of treatment in both groups was 16 weeks. The clinical efficacy and total cholesterol（ TC）,triglyceride（ TG）,fasting blood glucose（ FBG）,glycosylated hemoglobin（ Hb A1 c）,two hours postprandial blood glucose（ 2 h PBG）,low density lipoprotein cholesterol（ LDL-C）,high-density lipoprotein cholesterol（ HDL-C） in two groups before and after treatment were compared. Results The total effective rate of the observation group was higher than that of the control group[91. 67%（ 55/60） vs 78. 33%（ 47/60） ]（ P〈0. 05）. After treatment,the FBG,Hb A1 c,2 h PBG of the observation group were lower than those of the control group [（ 5. 22 ± 0. 39） mmol/L vs（ 5. 68 ± 1. 61） mmol/L,（ 5. 08 ± 1. 04） mmol/L vs（ 5. 49 ±1. 03） mmol/L,（ 7. 78 ± 0. 89） mmol/L vs（ 8. 13 ± 0. 84） mmol/L]（ P〈0. 05）. After treatment,TC,TG and LDL-C of the observation group were lower than those of the control group[（ 3. 96 ± 0. 35） mmol/L vs（ 4. 59 ± 0. 56） mmol/L,（ 1.63 ± 0. 58） mmol/L vs（ 2. 06 ± 0. 65） mmol/L,（ 2. 68 ± 0. 97） mmol/L vs（ 3. 17 ± 0. 63） mmol/L]（ P〈0. 01）. The HDL-C was higher than that of the control group[（ 1. 98 ± 0. 57） mmol/L vs（ 1. 79 ± 0. 46） mmol/L]（ P〈0. 05）. Conclusion Invigorating spleen and removing dampness and dredging collaterals treatment combined with conventional treatment for type 2 diabetic patients with abnormal lipid metabolism has a good effect,with better performance in improving blood lipid and blood glucose indexes than the conventional western medicine treatment alone.
