儿童肺炎链球菌感染对患儿T细胞亚群和血清免疫球蛋白水平的影响

Influence of streptococcus pneumoniae infection on T cell subsets and serum immunoglobulin levels in children

目的探究儿童肺炎链球菌感染对患儿T细胞亚群和血清免疫球蛋白(Immunoglobulins,Ig)水平的影响。方法选取50例2016年4月至2017年8月于该院接受治疗的儿童肺炎链球菌感染患者作为感染组,另选取30例在该院接受治疗的哮喘患儿作为哮喘组,20例于该院体检的正常儿童作为对照组。采用ELISA法检测白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-8(Interleukin-8,IL-8)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)表达;采用流式细胞术检测CD3^+、CD3^+CD4^+、CD3^+CD8^+、CD4^+/CD8^+表达;采用散射比浊法检测免疫球蛋白及IgG亚类(IgG subclass)表达。结果哮喘组和感染组患儿IL-6、IL-8、TNF-α均明显高于对照组差异有统计学意义(P<0.05);感染组CD3^+CD4^+、CD3^+CD8^+、CD4^+CD8^+表达水平均明显低于哮喘组和对照组,差异有统计学意义(P<0.05);患病组IgA、IgM、IgG1、IgG2、IgG3、IgG4表达水平均明显低于哮喘组和对照组,差异有统计学意义(P<0.05)。结论 T细胞亚群和血清免疫球蛋白水平在肺炎链球菌感染患儿体内存在异常表达,通过检测T细胞亚群和血清免疫球蛋白水平表达,可以提高儿童肺炎链球菌感染的检出率,对肺炎链球菌感染患儿的治疗及预后有重要的应用价值。

Objective To Explore pneumococcal infections in children with T cell subsets and the level of serum immunoglobulin （Immunoglobulins, Ig）. Methods Select 50 cases in April 2016 - August 2017 in our hospital for treatment of children with streptococcus pneumoniae infection as infection group, the other selected 30 cases in our hospital for treatment of children with asthma as asthma group, 20 cases in our hospital check-up normal children as control group. The expression of interleukin-6 （ IL-6 ） , interleukin-8 （ IL-8 ）, Tmnor necrosis factor-α （TNF-α） were detected by ELISA. The expressions of CD3 ＋ , CD3 ＋ CD4 ＋ , CD3 ＋ CD8＋, CD4＋/CD8＋ were detected by flow cytometry, hnmunoglobulin and IgG subclass expression were detected by scattering turbidimetry. Results II-6, IL-8 and TNF-α were significantly higher in asthma group and infection group than in the control group （P 〈0. 05 ）. The expression levels of CD3 ＋ CD4 ＋ , CD3 ＋ CD8 ＋ and CD4 ＋ CD8 ＋ in the infection group were significantly lower than those in the asthma group and the control group, and the difference was statistically significant （P 〈 0. 05 ）. The expression levels of IgA, IgM, IgG1, IgG2, IgG3 and IgG4 in the diseased group were significantly lower than those in the asthma group and the control group, and the difference was statistically significant （ P 〈 0. 05 ）. Conclusion T cell subsets and serum immunoglobulin levels in streptococcus pneumoniae infection in children with abnormal expression, by detecting the T cell subsets and serum immunoglobulin levels, can improve the detection rate of streptococcus pneumoniae infection in children, treatment and prognosis of children with streptococcus pneumoniae infection has important application value.
