支气管黏液栓形成对儿童肺炎支原体肺炎的影响

Effects of bronchial mucus plugs formation on mycoplasma pneumoniae pneumonia in children

目的了解支气管黏液栓形成对儿童肺炎支原体肺炎(MPP)患儿的影响。方法回顾性分析2012年6月至2014年12月134例行纤维支气管镜的MPP患儿的病史资料。根据纤维支气管镜下情况,分为黏液栓组和非粘液栓组。对2组患儿基本情况、临床特征进行统计分析。结果 2组患儿性别构成及年龄比较差异均无统计学意义(P>0.05)。2组患儿入院前病程及首次行支气管镜时病程比较差异均无统计学意义(P>0.05);黏液栓组患儿住院后热程和住院天数分别为(6.5±5.5)d、(19.4±6.7)d,均明显高于非粘液栓组[(2.4±2.4)d、(13.4±4.3)d],差异有统计学意义(P<0.05);黏液栓组高热及胸腔积液发生率较高,差异有统计学意义(P<0.05)。结论支气管黏液栓形成MPP患儿热峰高、胸腔积液发生率高,发热时间及住院时间延长。

Objective To investigate the effects of bronchial mucus plugs formation on mycoplasma pneumoniae pneumonia（MPP） in chidren. Methods The clinical data about 134 children patients with MPP who were treated in our hospital from June 2013 to December 2014 were retrospectively analyzed. According to examination results by flexible fiexible bronchofiberscopy,these patients were divided into mucus plug group and non mucus plug group. The clinical features and laboratory examination results were analyzed and compared between the two groups. Results There were no significant differences in patient＇s age,sex,the disease course before admission and the disease course in the first treatment by fiexible bronchofiberscopy（P〉 0. 05）. The fever duration and hospital stay in mucus plug group were significant longer than those in non mucus plug group（P〈 0. 05）. Moreover the incidence rates of pleural effusion and high fever in mucus plug group were significantly higher than those in non mucus plug group（P〈 0. 05）. Conclusion The chidren patients with MPP complicated by bronchial mucus plugs formation have the characteristics of high peak and longer duration of fever,higher incidence of pleural effusion and longer hospital stay.
