尿激酶对老年肿瘤患者输液港封管效果的研究

Effect of urokinase on sealing efficacy of the transfusion port of elderly tumor patients

目的探讨尿激酶对老年肿瘤患者输液港封管效果的影响。方法将70例使用末端开口输液港的老年肿瘤患者随机分为对照组和观察组,分别使用100 U/m L肝素盐水封管和5 000 U/m L尿激酶封管,比较2组导管连续输液和输液间歇期堵塞率、感染、出血、外渗情况以及凝血酶原时间(PT)值变化。结果连续输液期间,观察组导管堵塞率为1.74%,对照组为1.15%,2组差异无统计学意义(P>0.05);输液间歇期,观察组堵管率为6.83%,对照组为2.51%,2组差异有统计学意义(P<0.05)。2组感染、出血、外渗发生率差异无统计学意义(P>0.05)。2组化疗前及化疗3周期后PT值变化比较差异无统计学意义(P>0.05)。结论化疗间歇期用5 000 U/m L尿激酶2 m L封管能降低导管堵管发生率,使用肝素和尿激酶封管未增加老年肿瘤患者的出血发生率,可安全应用于临床。

Objective To investigate effect of urokinase on sealing efficacy of the transfusion port of elderly tumor patients. Methods A total of 70 elderly tumor patients with open-ended transfusion port were randomly divided into control group and observation group,given 100 U/m L heparin saline and 5 000 U/m L urokinase,respectively,for tube sealing. The blocking rate,infection,bleeding,extravasation,and changes of prothrombin time（PT） values during continuous infusion and intermittent infusion in two groups were compared. Results During the continuous infusion period,the catheter blocking rates were 1. 74% in the observation group and 1. 15% in the control group,but no significant difference between two groups was observed（P〉0. 05）. The catheter blocking rate was 6. 83% in the observation group and 2. 51% in the control group during intermittent infusion,which showed a significant difference in two groups（P〈0. 05）. No significant differences was found in the incidence of infection,bleeding,extravasation in both groups,nor in PT value changes before chemotherapy and after 3 courses of chemotherapy. Conclusion The use of 5 000 U/m L urokinase for 2 m L in tube sealing during intermittent chemotherapy can reduce incidence of catheter blocking,and the use of heparin and urokinase for tube sealing does not increase the incidence of bleeding in elderly tumor patients,so it is safe in clinic.
